- Request by Corp Regarding an Unpaid Tax Liability (needs to be completed and sent in with quote) Purchase Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is F3Q2AD4126A001 and this is a Request For Quotation (RFQ). This announcement is issued as a combined synopsis/solicitation. This requirement is FULL AND OPEN COMPETITION. The anticipated award will be a Firm Fixed Price Contract, the award will be made based on the lowest price. The solicitation shall incorporate provisions and clauses that are in effect through Federal Acquisition Regulation (http://farsite.hill.af.mil). Federal Acquisition Regulation 252.204-7004 (Alt A) requires Central Contractor Registration. All respondent offerors must be registered in https://www.sam.gov in order to be eligible for contract award. This announcement constitutes the only solicitation. Quotes are being requested, and a separate written solicitation will not be issued. THIS SOLICITATION WILL NOT RESULT IN AN ORDER AGAINST AN FEDERAL SUPPLY SCHEDULE. ANY OFFER WHICH IS PREDICATED UPON THE AWARD BEING PLACED AGAINST AN FSS WILL NOT BE CONSIDERED. SMALL BUSINESS SIZE STANDARD: (a) NAICS: 33923 (b) NR OF EMPLOYEES: 500 CLIN 0001 - Hydraulic resucue tool equipment system IAW the attached purchase description Price(s) should be FOB Destination (Robins AFB GA 31098) and payment terms (Net 30 Days). In addition to price and delivery, quote should include your cage code, DUNS number, and taxpayer ID number. Items are requested to be delivered NLT 3 weeks after receipt of award. Quotations should be emailed to Jimmy Ezzell (jimmy.ezzell.1@us.af.mil) no later than 20 June 2014, 3:00 pm EST. The anticipated award date is 1 July 2014. Please do not request award status prior to this date. The following clauses are applicable: FAR 52.204-99 System For Award Management Registration FAR 52.212-1 Instructions to Offerors FAR 52.212-2 Evaluation-Commercial Items FAR 52.212-3 Offeror Representations and Certifications - Commercial Items FAR 52.212-4 Contract Terms and Conditions - Commercial Items FAR 52-212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, and DFARS 252.212-7001 FAR 52.247-34 FOB Destination DFARS 252.232.7003 Electronic Submission of Payment Requests (WAWF is the method used by DoD for processing invoices/receiving reports.) In accordance with Clause 52.212-2, the evaluation factors shall be price and price related factors. And in accordance with the RFQ DFARS 252.209-7995 Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law (Attached to be filled out) See Class Deviation 2012-O0004, Prohibition Against Contracting With Corporations That Have an Unpaid Delinquent Tax Liability or a Felony Conviction under Federal Law, dated January 23, 2012. Contracting officers shall include this provision in all solicitations that will use funds made available by Division A of the Consolidated Appropriations Act, 2012, including solicitations for acquisition of commercial items under FAR part 12, and shall apply the restrictions included in the deviation. This deviation is effective beginning January 23, 2012, and remains in effect until incorporated in the FAR or DFARS or otherwise rescinded.
