Description

The purpose of the service is to provide Community Nursing Home Care (CNHC) services in the ELKHART IN area for beneficiaries for whom such care is specifically authorized by the VAMC. The service is in support of the Social Services Department at the NIHCS VAMC. Prices provided for Community Nursing Home Care Services are under the Indiana Medicaid program for nursing home's case mix rate RUG III as listed on their most recent Myers & Stauffer Medicaid Rate Letter. The percentages remain the same during the full term of the BOA. Rates shall be inclusive of all services as may be necessary in the treatment of the veteran. Services shall be provided for a period of twelve (12) months from date of award with four (4) one-year option periods to be exercised at the discretion of the government. The minimum value of services and maximum value of services to be provided under this contract during each contract year is as follows [note - minimum value of services is only applicable to option years if the VA, in its sole discretion, elects to exercise the option for that year]: Description of CarePer Diem Rate VA Reduced Physical Function - RUG III: 12 & 13 VA Basic - RUG III: 5, 6, 9, 10, 11, 14, 15 & 16 VA Heavy Rehabilitation Therapy - RUG III: 1 & 2 VA Special Care - RUG III: 3 & 4 VA Clinically Complex - RUG III: 7 & 8 VA Ventilator Dependent VA HIV/+AIDS VA Special Bed-Dependent
