- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The South Dakota Army National Guard (SDARNG) has a request to provide job-related Medical Surveillance Examinations and Occupational Health Services to the higher risk full-time support (FTS) and/or temporary personnel who are or could potentially be exposed to health hazards in the work environment. The examination shall include personnel assigned to positions requiring specific standards of physical fitness. There are 2 locations that require this service: CSMS-1 Mitchell, SD, 57301 and CSMS-2 Rapid City, SD, 57701.All medical surveillance testing desired to be complete in July 2014. No award shall be made unless the Contracting Officer makes confirmation or affirmative determination of responsibility for the potential contractor. Potential contractor must be registered in System for Award Management (SAM) prior to award of contract. This acquisition shall be 100% Small Business set aside. Applicable NAICS 621399 for Offices of All Other Miscellaneous Health Practitioners, the small business size standards is $7.0M. RFQ solicitation response date is 3 June 2014.
