- Past Performance Survey The purpose of this admendment is to upload the past performance survery, extend the dead line submission date from June 2, 2014 to June 6, 2014 and post questions and responses.The point of contact email should be rodney.brooks@nih.gov. 1. The Announcement stated the following: All responses must be received by June 2, 2014, 12:00 p.m., eastern standard time, and must reference number HHS-NIH-NIDA-SSSA-2014-212. Responses may be submitted electronically to Rodney Brooks, Contract Specialist at rodney.brooks@.nih.gov or by U.S. mail to the National Institute of Drug Abuse (NIDA), Station Support / Simplified Acquisition Branch (SS/SA), 31 Center Drive, Building 31, Room 1B59, Bethesda, Maryland 20892-2080, Attention Rodney Brooks., Yet the Quote Instructions states: Submit one (1) and four (4) copies. Question: Would the government consider receiving electronic submissions as stated in the announcement versus hard copies as stated in the Instructions? YES 2. The Instructions state that the Offeror shall be responsible for requesting three (3) references to submit the Past Peformance Information Survey Questionnaire (see Attachment B) to the Contract Specialist no later than February 7, 2014, 2:00 pm EST at kantwi@hrsa.gov. Question: Would the government provide the updated instructions on how offerors should address Past Performance? ATTACHED IS THE PAST PERFORMANCE SURVEY 3. The Statement of Work seems to be missing data and/or includes instructions to the person who prepares the document leading to confusion. Question: Would the government consider providing a clean version of the SOW? NO FOLLOW INSTRUCTIONS THEY ARE CLEAR. 4. Question: Would the government consider extending out the due date beyond the current June 2nd due date? YES THE SUBMISSION DATE HAS BEEN EXTENDED TO JUNE 6,2014.
