Description
A.GENERAL INFORMATION 1. Description of Project: Exercise Class for Veterans - The cardiac wellness program will be a modified version of our current cardiac rehab program. Participants will require monitoring while using exercise equipment (treadmill, stepper, stationary bike). Each week their exercise routine adjusted as tolerated. Cardiac rehab staff will be present in immediate area. Eligible participants for the cardiac wellness program will be patients enrolled in the MOVE weight loss program that have cardiovascular risk factors (e.g. Diabetes, Hypertension, Hyperlipidemia) but without prior cardiac events. Looking for an exercise instructor that is: oAble to evaluate a person's overall health, with special attention to cardiovascular function and metabolism oAble to take vital signs oKnowledgeable with the use of exercise equipment oNeeds to have certifications in: "BLS certified "Cardiac Rehab certified oAble to work with patients to provide physical fitness, strength, endurance, and flexibility within the parameters of the exercise prescription as ordered by physician The class will take place 3 times per week in Jamaica Plain, MA in Cardiac Rehabilitation Room 11-C-31 2. Scope of Work: The contractor shall provide all resources necessary to accomplish instruction at three sessions (M-W-F classes) per week at 2 hour per session for 40 weeks. The class sizes maximum is 10-12 participants. 3. Background: This exercise program was created as a result of the 2014 Veterans Integrated Service Network (VISN) Innovation Project Grant. 4. Performance Period: The period of performance shall 40 weeks. The work at the Government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer's Technical Representative (COTR). B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this Performance Work Statement (PWS) until the Contracting Officer has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide a specific description of the ability to provide physical fitness, strength, endurance, and flexibility in addition to the ability to instruct and safely monitor patients while exercise equipment within the parameters of the exercise prescription D. SCHEDULE FOR DELIVERABLES 1.At the time for quote submittal, the contractor identify by name, the key personnel who will work under this task order as well as the certifications identified in Section A.1. 2.The contractor shall also provide the items identified in Section C. 3.The period of performance shall be three classes per week for 40 weeks. E.CHANGES TO PERFORMANCE WORK STATEMENT Any changes to this PWS shall be authorized and approved only through a signed modification by the Contracting Officer. F. GOVERNMENT RESPONSIBILITIES The Veterans Administration is responsible to provide a space/site for the exercise program for veterans. The exercise sessions will take place in cardiac rehabilitation space. G. CONTRACTOR EXPERIENCE REQUIREMENTS Qualifications of Key Personnel The contractor shall identify by name, the key personnel who will work under this task order. The government will not dictate specific experience and education requirements of the key personnel to perform the work stated herein. The contractor shall submit a resume of qualifications for the COTR. All Contractor employees will be approved by the COTR prior to bringing on duty. If, at any time from date of award to the end of the contract, Contractor personnel are no longer available, the COTR will approve the replacement prior to bringing on duty. The contractor must inform the VHA COTR, and Project Manager/Task Manager when personnel are removed from the contract for any reason. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons.
