- map map map map map map quote doc and clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation AG-52B1-S-14-0018 is being issued as a Request for Quotation (RFQ), to perform Birdseye habitat improvement services on the Hoosier National Forest. The work will be performed in Crawford County, IN. The work consists of bush-hogging and grass/forb identification. This acquisition is set-aside for small businesses. The North American Industry Classification System (NAICS) code is 115310 and the size standard is $7mil. Work will begin August 15, 2014 and be completed by November 15, 2014. BASIS FOR AWARD: a single award will be made to the contractor based on best value to the government. Offerors shall provide both pricing and technical information. Pricing and technical information will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are acceptable at rkmanning@fs.fed.us. Service Contract Act wage determination is applicable. Value is estimated to be less than $100,000. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically at http://orca.bpn. gov., submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial Items is applicable with the following factors used to evaluate quotes: Price, technical conformity to specifications, and past performance. Clauses indicated on a separate attachment.
