- This is a Sources Sought Notice; there is no solicitation available at this time. Requests for solicitation will receive no response. This Sources Sought Notice is published for market research purposes only. This Sources Sought Notice is conducted to identify potential sources capable of meeting the requirements described below. The Lebanon Veterans Affairs Medical Center (LBVAMC) located at 1700 South Lincoln Avenue, Lebanon, PA 17042 requires primary care services for eligible veteran patients in the Schuylkill County area of Pennsylvania via the establishment of a community based outpatient clinic (CBOC). The current CBOC in Schuylkill County has approximately 2,400 enrollees. The contractor shall furnish health care providers, medical facilities, equipment and supplies, emergency-fill pharmaceutical prescriptions, and administrative functions to fulfill the level of care and range of services necessary to meet the primary medical needs of enrolled patients. The contractor must be accredited by Joint Commission or maintain a level of service that is in compliance with all current Joint Commission standards. The services covered by the contract shall be furnished by the contractor Monday through Friday 7:30 am to 4:00 pm with telephone triage available 24 hours a day, seven days a week. The contract period will include a one base year and four one-year option periods. The applicable NAICS Code is 621498 and the size standard is $19.0M. Responses are due by 12:00 PM EDT on Thursday, 22 May 2014 via email to Rachel.Turk@va.gov. Telephone responses will not be accepted. Please do not make contact with any other LVAMC staff member regarding this acquisition. ADDITIONAL INFORMATION: The Government will neither award a contract solely on the basis of this notice nor will it reimburse vendors for any costs associated with preparing or submitting a response to this notice. This notice does not constitute a Request for Quotation (RFQ), Request for Proposal (RFP) or an Invitation for Bid (IFB), nor does this issuance restrict the Government's acquisition approach. The Government reserves the right to reject in whole or part any industry input as a result of this announcement. The Government recognizes that proprietary data may be a part of this effort. If so, clearly mark such restricted or proprietary data and present it as an addendum to the non-restricted/non-proprietary information. If the Government decides to award a contract for this or related effort, a solicitation will be published and posted on the FedBizOpps website.
