Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-VA-251-14-Q-0319 is issued as a request for quote (RFQ), and is a set-aside for small business under NAICS code 424720 (500 employees). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. No telephone requests will be accepted. Only written or email requests received directly from the requester are acceptable. Line Item 1 - Contractor shall provide 10,000 gallons of premium ultra low sulfur diesel fuel to be used for the Ann Arbor VA Medical Center. This fuel is for generators and boilers in house in the Energy Center at Building 32 located at 2215 Fuller Road, Ann Arbor, MI 48105. Please see attached Statement of Work. Offorers must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a purchase order resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Price will be evaluated to determine its fairness, completeness, and reasonableness as it relates to the offer. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4,7, 25, 27, 36, 37, 46, 48. The following additional clauses apply: SAM Clause: Attention: You must be registered in SAM to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, its DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the SAM database. (C) Lack of registration in the SAM database will make an offeror ineligible for award. Offerors and contractors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via the Internet at https://www.SAM.gov. Electronic Funds: FAR 52.252-2, Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. 852.232-72, Electronic Submission of Payment Requests, 852.270-4, Commercial Advertising. Contact Kelly Poole, Contract Specialist, at Kelly.poole@va.gov. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT I.SCOPE OF WORK: The Contractor shall provide fuel for Ann Arbor VA Healthcare Facility. Location for the fuel drop off will be Ann Arbor VA Medical Center, 2515 Fuller Road, Ann Arbor, MI 48105. This contract is for fuel for the Ann Arbor Facility. The contractor will perform all necessary work to fill fuel tanks with Premium Low Sulfur Diesel Fuel for Building 22 to be used to top off the tanks for the facility Generators and Boilers in the amount of 10,000 gallons. II.CONFORMANCE STANDARDS: All services provided under this contract must be performed in conformance with the Federal regulations and AAVA policies and requirements. III.PREVENTIVE MAINTENANCE: All preventive maintenance will be done by the contractor through EPA, OSHA and VA Ann Arbor Medical Center provisions. IV.REPORTING REQUIREMENTS: The Contractor shall be required to report to the Facilities Management Energy Center Office at the Ann Arbor VA Medical Center to log in with the Energy Center Section during normal working hours. This check in is mandatory. When the service is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out at the Facilities Management Office Grounds and Transportation Section and submit the field service report to the Contracting Officer Technical Representative for a signature, during normal working hours. If the Contracting Officer Technical Representative is unavailable, a signed authorized copy of the field service report shall be sent to the Contractor after the work has been reviewed. V.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
