- W-9 Vendor Questionnaire Scope of Work Pest Control Blanket Purchase Order Opportunity for Pest Control Service. We are currently soliciting bids for a Blanket Purchase Order (BPA) for the Gadsden Job Corps Center. Our option begins June 1, 2014 to November 30, 2014. Please see below the scope of work and service: Scope of Work: 1. Provide pest control services on a monthly schedule. 2. Special pest control services for the winter break in the male and female dorms. 3. Provide pest control services, as needed, for the control of ants, bees, wasps, and mice that may infest the center. 4. Provide an annual termite/carpenter ant inspection of all buildings. Service will include: ▪ academic building ▪ administration building (2 floors) ▪ SIB building ▪ cafeteria ▪ gym ▪ male & female dorms ▪ outside area around all buildings ▪ other areas on center as needed 5. Available for recall at no additional expense to the Center in the event that the service did not resolve the pest problem in adequate time.
