Description

Introduction/Background The VA New York Harbor Healthcare System (VA NYHHS) is dedicated to providing quality health care to veterans using the abilities of all employees supported by our commitment to education and research. We are recognized locally, regionally and nationally as a leader in quality patient care, positive customer service, medical/allied health education, health related research and employment opportunities. We seek to be the provider of choice or veterans and the community by offering an efficient, integrated quality health care system capable of providing a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change. Our key business drivers are quality, financial integrity, patient satisfaction and employee satisfaction. Our library service for veterans and staff contains a collection of easy?to?read health and wellness books, respected health education newsletters, videos and other materials. Objectives The library service has been hard at work to modernize library resources. The goal of the library service is to provide staff and veterans access to a host of new products including: new consumer health books, DVDs, audio books, and new subscriptions to numerous sports, entertainment, political and health magazines. The new design will feature an open layout that includes multiple VA NYHHS VA Medical Center employee computer workstations, few lounge chairs with laptop tables and a laptop docking table. The interior room of the facility has been converted to a patient library, complete with two computer workstations and numerous chairs with laptop tables. Additionally, the library service also plans to purchase additional computer workstations and a printer for patients' use. Scope of Work The contractor shall: "Furnish and install approximately 36 linear feet of Clipper Partition Range A, Single Glazed Partition System "Ceiling height is approximately 90" "Partition is glazed with 1/2" clear tempered glass "Installation includes (4) framed butt hinge doors with drop seal for best acoustic performance "Framed doors to be glazed with 5/16" clear tempered glass "Framed doors to be furnished with non?locking lever sets. "All extrusions will be manufacturer's standard silver powder coated paint, reference Aluminum Interpon AKZO Nobel. Samples will be provided. "Glass fronts are to be butt joint (glass to glass) connections joined by a 1/8" thick 3M transparent adhesive gasket. "Base channel is 1?7/8" high and secured to hard floor or carpet. "Top channel is 2 ½" high and secured to dropped ceiling. "Nonunion installation and Clipper Technical Specialist for duration of the project included. "Pricing includes materials and installation for 24" wide band of 3M Dusted Crystal privacy film across all glass "Shop drawings will be furnished within 1 week of receipt of signed proposal The contractor will work with the VA NYHHS Environmental Management Services (EMS) and Engineering Service (ENG) to determine space forecasting and building analyses. Period of Performance Contract staff will be required to work five days a week, 8 hours a day excluding weekends and federal holidays unless directed by the Contracting Officer or Contracting Officer Representative (COR) Requirements of this contract will be fulfilled within thirty (30) days from award of contract. Place of Performance Services will be provided at the VA NYHHS Library Center VA New York Harbor Healthcare System Library Service - 2nd Floor North 423 East 23rd Street New York, NY 10010 Compliance The contract(s) will be compliant with the Joint Commission Accreditation of Health Care Organization (JCAHO) and the VA Medical Center Regulations. Billing/Payments The EMS and ENG service shall establish and maintain a record keeping system that will record the hours worked by the contractor employee(s). Contractors shall report to the COR or designees upon arrival at the facility. Invoices must be submitted to the COR delegated to this contract as well as submitted as directed on the approved contract. The COR or designee will maintain all invoices and will review them for accuracy prior to payment. Security Responsibilities The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse the VA within 30 days. Performance Monitoring Performance and progress will be monitored and tracked by the designated COR and reported regularly to the Contract Officer and VA NYHHS Management.
