Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. The Department of Veterans Affairs, Syracuse VA Medical Center, Radiology, 800 Irving Avenue, Syracuse, New York, 13210-2716 intends to award a firm, fixed price services contract to perform Cryoablation cancer treatment procedures utilizing the percutaneous interventional radiology method to treat Renal Cell Carcinoma. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Submission of proposals must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. The Government will award a firm, fixed-price contract to the responsible Offeror whose offer conforming to the solicitation provides the Best Value to the Government, price and other factors considered. The applicable NAICS code is 423450 and the size standard is 100 employees. This is 100% set aside for Small Business. The VA will not pay any costs incurred in preparation and submission of proposals. Offerors shall include a completed copy of the provision FAR 52.212-3 Offeror Representations and Certifications-Commercial Items which is completed by registering in the System for Award Management (SAM) Website at: https://www.sam.gov/portal/public/SAM/. Proposals must also include contractor's DUNS number, prompt payment terms, and correct remittance address. OFFERORS MUST BE REGISTERED WITH SAM PRIOR TO CONTRACT AWARD. The solicitation package will be available on or about May 7, 2014. The date for receipt of proposals is May 19, 2014 at 2:00 PM EDT. All technical and price proposals must be submitted electronically (e-mailed) to richard.williams18@va.gov.
