- The Veterans Health Administration at Wilkes-Barre, PA is seeking small business vendors who can deliver Nuclear Medicine per the following Statement of Work. STATEMENT OF WORK F-18 and F-18 Na-F Isotopes 1. General Description: The purpose of this contract is for radiopharmaceutical preparation and delivery of F-18 and F-18, Na-F doses. Approximate usage is 20 doses per week for F-18 (although this is an indefinite guaranteed quantity) and two doses per week of F-18,NaF., Testing performed at Contractor facility provided by a board-certified radiopharmacist and delivered to the VA nuclear medicine department at 1111 East End Blvd. Wilkes-Barre, PA. Deliveries will be made Monday through Friday at 7:30am. Standard daily delivery times will be 7:30am. In the event a second delivery is necessary, which may occur less than 5 times per month this delivery will occur prior to 2:00pm the same day. Patient name and last four social security numbers will be provided with the order. The contractor is responsible for maintaining patient information in the strictest confidence in accordance with HIPPA and other privacy regulations. All isotope products will be clearly bar coded and compatible with Nuclear Medicine Information System (NMIS). Bulk /unit doses delivered will be adequately shielded in accordance with DOT regulations and packaged to prevent damage during transportation. Bulk doses will be provided when three or more doses are ordered. Delivery vehicles and staff must comply with federal/state and local regulations. The contractor must meet all NRC, NHPP and DOT regulations and will submit evidence of current license and registration. The contractor shall pick up day's unused dosages for proper disposal. Contractor shall provide emergency deliveries as requested by the VA nuclear medicine department. Contractor shall require all of its personnel be licensed in accordance with all applicable laws and regulations prior to the effective date of performance under this Contract. All doses will meet quality control regulations required by state and federal regulatory agencies. Unused doses will be decayed and returned to the vendor on next delivery day for proper disposal by the contractor. The below radiopharmaceutical products are needed to meet the agency's need, however request for radiopharmaceuticals is not limited to this list. Estimated quantities below are for a period of twelve months. Product Est. Qty F-18 FDG Isotopes 20 doses per week F-18 NaF Isotopes 2 doses per week 2.Clinical Duties: Contractor's employees will be given access to the rear entry of the nuclear medicine department at 7:30am there is always a staff member available to provide access. All deliveries will be placed directly in the "hot lab" room 209 in nuclear medicine. 3.Administration: a.Contractor will invoice monthly in arrears. b.All services rendered will be itemized by date of service, radiopharmaceutical provided and unit cost. 4.Security and Confidentiality: a. Responsible for maintaining Automated Data Processing (ADP) security. b.Responsible for maintaining patient information in the strictest confidence in accordance with the Healthcare Insurance portability and Accountability Act (HIPAA) and all other applicable privacy rules and regulations. 5.Service Hours: a.The service covered by this contract shall be furnished by the Contractor and Defined herein. The Contractor shall not be required, except in case of emergency, to furnish such services on a national holiday or during non-service hours as described below. b.Service hours: Monday through Friday, 7:30am - 4:30pm. 6.Delivery Orders: Orders normally will be placed by 4:30pm and delivery shall be required by 7:30am the following work day. Due to the demands of the Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required per day. Items shall be adequately packaged to prevent damage during handling and storage. The delivery order shall specify: contract item(s) and quantity, time(s) of delivery and applicable VA order number. Orders can be placed orally, electronically, or by telephone and/or fax transmission. 7.Delivery Location: VA MEDICAL CENTER VA Nuclear Medicine Department 1111 East End Blvd. Wilkes-Barre, PA 18711 oDeliveries will be made Monday-Friday (excluding holidays which are listed below) between the hours of 7:30am and 4:30pm, except in case of emergency. oAccess to this location during non-business hours shall be determined at the time of the order. 8.Delivery Commitment: Time of delivery specified at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. 9.Person Authorized to Place Orders: Any Certified Nuclear Medicine Technologist employed by the VA Medical Center, Wilkes-Barre, PA. 10.Federal Holidays: Federal Holidays, during which regular deliveries are not required, include: oNew Year's Day oBirthday of Martin Luther King, Jr. oWashington's Birthday oMemorial Day oIndependence Day oLabor Day oColumbus Day oVeterans Day oThanksgiving Day oChristmas Day oAny other date so designated by executive order 11.Regulatory Compliance: Contractor shall meet all Nuclear Regulatory Commission, Department of Transportation, F.D.A. and all other agency rules and regulations (Federal and State). Contractor shall perform all quality control procedures as required by Federal and State regulatory agencies. 12.Quality Control: All products offered to the VA must be of the same consistent high quality as would be offered to the general commercial public in the United States. a.Any substitution of product shall have the prior written approval of the VA Nuclear Medicine Service and the Contracting Officer or his/her designee. b.All defective or inferior products shall be replaced within 48 hours at no costs to the VA. c.It is the Contractor's responsibility to provide the VA with recall notices on any product that has been delivered to the VA that may be defective. Notification shall be made to the COR via the most expedient method. 13.Period of Performance: This solicitation is for one year (base year) with four option years. 14.Contracting Officers Representative: Mary Jean Kirschner, 570 824-3521 x7452. -END OF STATEMENT OF WORK-
