Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0301 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 339112 and the business size standard is 500 employees. Requestors shall state their business size on their proposal. Proposed items must be BRAND NAME OR EQUAL to the items requested below. They must perform to exact specifications of the brand names referenced (Modus Medical Devices, QUASAR Programmable Respiratory Motion Phantom).Contractor may be asked to prove the performance and characteristics through schematics or demonstration, to be scheduled with the Contracting Officer. All responsible sources may submit a proposal which shall be considered by the contracting officer.
