- Request for Quote Package SF-18, Request for Quotation The Health Resources and Services Administration (HRSA), National Hansen's Disease Programs (NHDP), is contemplating issuance of a purchase order pursuant to Federal Acquisition Regulations (FAR), Part 13. The purpose of this procurement is to purchase Occupational Therapist Services for the treatment of Hansen's Disease (HD) patients located in located at Ochsner Hospital Baton Rouge, LA, at the Saint Clare Manor nursing home, and/or at the NHDP facility in Carville, LA. The Contractor shall perform the following tasks: 1. Evaluate and treat patients, perform hand screens as needed, and refer patients within one (1) day to specialty clinics with hand problems and other functional needs. The Contractor shall implement appropriate treatment according to findings. 2. Fabricate customized plaster casts and thermoplastic splints to promote wound healing, increase range of motion, prevent injuries and to increase function the same day the patient is scheduled. 3. Evaluate patients' home environment to minimize and prevent injuries and to enhance their independence and functional status. 4. Consult with National Hansen's Disease Program (NHDP) Nursing Department and Social Services regarding functional and social concerns of patients and make appropriate recommendations. 5. Consult with attending physicians and collaborate with other staff regarding evaluation results and treatment goals. 6. Evaluate patients' adaptive living skills and provide treatment and assistive devices as appropriate. 7. Design orthotic devices to assist patients in reaching their optimum level of function. 8. Assist in maintenance of inventory by tracking materials and supplies used in the Occupational Therapy department and each month provide the Contacting Officer Representative (COR) an inventory of items that need to be restocked. 9. Assist in educating and training of visiting health care providers in the evaluation and treatment of the neuropathic patient and in prevention of injury and disability. 10. Perform cleaning and sterilization for approximately 20 packaged instruments and 50 unpackaged instruments per week for the Rehabilitation and Education Branch. 11. Provide assistance to occupational and physical therapy departments in maintaining and fabricating stock of monofilaments, and other frequently used clinical items (i.e. linen inventory). 12. Provide a monthly report to the COR of the number of hand screens completed, number of instruments sterilized and number of splints and pieces of adaptive equipment provided to the patients. 13. Collect data during patient evaluations and assist with the analysis and publication of results. The contractor shall provide up to 21 hours per week involving 3 days per week (7 hrs. a day) (except Saturdays, Sundays and Federal Holidays). The period of performance will be for a base period of twelve (12) months with one (1) twelve (12) month option period. The North American Industry Classification System (NAICS) for this requirement is 621999. The proposed simplified acquisition action is for supplies or services for which the Government intends to issue a solicitation under authority of the Federal Acquisition Regulations (FAR), Part 13. Responsible parties may submit a quote, which must be received within 15 days after the date of publication notice. This is a 100% Small Business Set-Aside.
