- This is not a request for proposal or quote announcement. This is a request for information only, do not submit pricing. The Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification relative to NAICS code 424210. In accordance with the VA's Veterans First Contracting Program, VA is interested in identifying Services-Disabled Veteran-Owned Small Business and/or Veteran Owned Small Businesses with the capability to perform. All interested and capable respondents are requested to provide the information below. Company name, address, point of contact, phone number, business size, DUNS number, list of available products and the geographical location the respondent is able to service. Responses to this notice should be sent electronically to Georgiann.schneider@va.gov. Telephone responses will not be accepted. Responses must be received by May 7, 2014. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database. The website for registration is www.SAM.gov. Service-Disabled Veteran Owned Small Businesses should also indicate the company is verified by the VA in the Vendor Information Pages (VIP) Database at https://www.vip.vetbiz.gov/ Statement of Work for VA Jesse Brown, Chicago, IL. 1.Contractor must provide daily routine deliveries and pick-ups as well as emergent deliveries and pick ups. Routine Weekday (M-F) delivery times: 5:00A or 6:00A (first daily delivery) and 10A (2nd daily delivery) and 1PM (3rd daily delivery). There are no scheduled weekend deliveries. However, VA may require Emergency orders within two hours of receiving the order by phone or otherwise from VA. VA has averaged approximately four emergency orders per month. (VA may prepare its own emergency kits for MAA and Mebrofenin.) 2.Contractor shall replace any items delivered in error at no cost to the VA. 3.Contractor shall label all products delivered as to the Unit and size of radiopharmaceutical, time of preparation, expiration time and date. 4.Vendor shall be in compliance with USP 797 guidelines and be able to show a plan for complete compliance upon award. 5.Contractor shall meet all Nuclear Regulatory Commission, Department of Transportation, F.D.A., and all other agency rules and regulations (Federal and State). Contractor shall perform all quality control procedures as required by Federal and State regulatory agencies. 6.Items must be adequately packaged to prevent damage during handling and storage. All packaging and labeling must meet all federal and state regulations. All packaging of radioactive material MUST BE clearly labeled RADIOACTIVE on the outside of the carton in which it is shipped. 7.Before contract award, Contractor shall provide a copy of their laboratory's current quality control manual to the Contracting Officers Technical Representative. 8.Contractor shall test all products for Quality Control periodically this testing shall be performed at no cost to the VA. The contractor shall provide a detailed description of the test upon request. 9.Contractor shall assign an individual sales representative to aide in facilitation in the smooth coordination between the medical center and Contractor's laboratory pharmacy. 10.Contractor shall pick up Radiopharmaceuticals for proper disposal at no cost to the Government. 11.Contractor shall replace, after written notice from the Lead Nuclear Medicine Tech any expired cold kits 10 days prior to its expiration date at no charge to the VA. 12.Contractor shall provide current material safety data sheets and updates on all delivered products for each medical center 13.Contractor shall provide patient orientation education video presentations and other appropriate material at no cost to the VA 14.Contractor shall provide signs and posters alerting patients, employees and the public, of safety precautions in handling of radioactive materials. 15.Any cost to replace damaged or incorrect products will be at the cost of the contractor. Replacements must be received within 48 hours of notification by the VA. 16.Contractor shall be licensed by the State of Illinois. Additionally, contractor shall be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radionuclides. Offeror must provide copies of these licenses and certifications to the Contracting Officer prior to award.
