- Equipment Maintenance Consolidation (Equipment List) Equipment Maintenance Consolidation (Sources Sought Notice) * See attached documents for detail. General Overview: The Uniformed Services University of the Health Sciences (USU), located in Bethesda, MD, has a requirement to consolidate a number of annual equipment maintenance services. Please carefully read the following and let us know if you are able to fulfill all or part of our requirement. This is a sources sought notice, not a request for proposal (RFP). Offerors responding to this notice will not be reimbursed for any costs associated in the preparation of their submission. The USU is the only Federal health sciences university and we are committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense (DoD) and the United States Public Health Service (PHS) and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU and its mission can be found on our website: www.usuhs.mil. If interested in this acquisition opportunity, please registered on SAM.gov with completed Representations & Certifications (Reps & Certs) using NAICS Code 811619 Other Electronic and Precision Equipment Repair and Maintenance with a size standard of $19,000,000. For those interested vendors, please review and complete the attached Equipment List spreadsheet columns O and P (and Q as necessary). Minimum Specifications: * Please refer to the attached Equipment List spreadsheet for details on the equipment components, annual PM, period of performance (PoP), and previous/existing purchase order (PO) information. Our intent is to award a single or multiple firm-fixed-price (FFP) option contract(s) for one base year + four option years (five years total). We understand some aging equipment units may not be covered for all five years. If the coverage cannot be extended for all five years, please note so in the column Q Vendor's Comment section. The maintenance service, including PM services, must be completed in accordance to the OEM's specifications. We will request a Certificate of Conformance to Manufacturer's Specifications and Operability from the awardee(s) at the end of the contract PoP for each equipment. A template Certificate will be issued at a later time, likely upon the issuance of the RFP. Interested vendors must offer unlimited remedial repairs and must be available during normal Federal work hours: Monday through Friday, excluding Federal holidays, from 7:30A to 4:00P. Vendors must be able to respond to any service call within 72 hours. Also, vendors must initiate and coordinate with end-users on all PM visits. All the equipment units are located in Bethesda, MD or its vicinity. The services must be completed onsite. Please address the following in your response: 1.Can you service all or some of our required equipment for five years (one base + four option years)? Please respond by completing column O of the attached Equipment List spreadsheet. If you cannot maintain our equipment for the full five years, please state the number of years you can offer under column Q of the spreadsheet. 2.Can you offer firm-fixed price? Please estimate per year price and complete column P of the attached Equipment List spreadsheet. Responses are due by 10:00PM EST on May 9, 2014. All responses must be emailed to the contract specialist, Andrew Lee, at andrew.lee@usuhs.edu. In your submission, please provide the following information: a.Official Company Name; b.Point-of-Contact, including name, email address, and phone number; and c.DUNS Number.
