Alaska Wage Determinations Attachment 5 - Mandatory DFARS Provisions Attachment 4 - FY 14 Passenger and Cargo Fees Attachment 3 - Alaska_Required Inspections and Schedule Attachment 2 - Instructions for Completing the AF Form 1222 Attachment 1 - AF Form 1222 - AF Equivalent (7-12-13) PWS Dated 15 April 2014 Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation will not be issued. The solicitation reference number is FA8051-14-T-0008 and is issued as a Request for Quote (RFQ). The 772 ESS/PKD Contracting Flight, Tyndall AFB, Florida, requires the following: Boiler and Unfired Pressure Vessel (UPV) Inspection services at Elmendorf AFB, Eareckson AFS, and King Salmon, Alaska. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-72 effective 30 Jan 2014, Defense Federal Acquisition Circular Notice DPN 20140421 effective 21 Apr 2014, and Air Force Acquisition Circular AFAC 2014-0421 effective 21 Apr 2014. This acquisition is being competed full and open under the North American Industry Classification System (NAICS) code 541330, Engineering Services. A firm-fixed price contract will be awarded. The contract will be awarded with two (2) Contract Line Item Number (CLINs). CLIN Description Qty Unit of Issue 0001 Boiler Inspection Services at 1 Job Elmendorf AFB, Eareckson AFS, and King Salmon, Alaska, Alaska June 2014 0002 Boiler Inspection Services at 1 Job Elmendorf AFB, Alaska July 2014 Award will be made to the lowest price offeror that is determined to be technically acceptable. To be determined technically acceptable, the proposal must meet the requirements of the attached Performance Work Statement (PWS). The offeror shall submit the Required Inspections and Schedule document (Attachment 2). Please note within Attachment 2, there are certain inspections that must be done on required dates. There are inspections that have open dates (noted by the term "variable"), which the offeror must account for. The schedule must be submitted with the offer and take into account the required inspection dates and the variable inspections. The schedule submitted with the offer will be binding. To show the inspectors are qualified to perform the inspections in accordance with the PWS, the offeror shall submit the proper documentation to show the inspectors meet the requirements of the National Board of Boiler and Pressure Vessel Inspections and the National Board Inspection Code (section 2.2 of the PWS). Any offer deemed technically unacceptable, will be eliminated from competition. The Government also reserves the right to make no award at all. Additionally, all contractors shall complete DFARS clause 252.209-7993 (see attachment 5); failure to do so will deem proposal ineligible for consideration. Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response. All offers must list your DUNS number, CAGE code, and Federal TIN with Company name, POC, and phone number. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official.
