- Quote Fill in Table Terms and Conditions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is HSTS04-14-Q-CT4037. This Request for Quotation (RFQ) is issued for a Blanket Purchase Agreement (BPA) for ADA Gates and Barriers. Offerors are requested to review the attached BPA Terms and Conditions and complete the Quote Fill in Table attachment. The Quote Fill in Table represents the offerors price quote submission. Supporting literature is requested to be provided. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-58. This acquisition is 100% set-aside for small business concerns. The North American Industry Classification System (NAICS) number is 332999. Small Business size standard is 750 employees. List of contract line item number(s): See attached BPA Terms and conditions. Description of requirements for the items to be acquired: See attached BPA Terms and conditions. Date of Delivery: See attached BPA Terms and Conditions. Place of delivery and performance: See attached BPA Terms and conditions. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, however no alternative terms and conditions or commercial items outside those specified in the attached BPA Terms and Conditions will be considered. The provision at 52.212-2, Evaluation -- Commercial Items, does apply. The evaluation process used is tradeoff process to determine best value as described in the attached BPA Terms and Conditions. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
