1 Introduction Naval Medical Center San Diego's Integrated Health Community Initiative (IHCI) has created a comprehensive population health strategy, connecting primary, secondary, and tertiary prevention to achieve optimal heath throughout the entire San Diego Multi-Service Market. 1.1 Sources Sought Response Criteria This Sources Sought does not constitute a Request for Quote (RFQ). This Sources Sought is issued for information and planning purposes only and does not constitute a solicitation. The Government will NOT make an award of a contract on the basis of this Sources Sought or otherwise pay for information received in response to this Source Sought. Do not provide proprietary information. Responses to this Sources Sought will not be returned nor will the Government confirm receipt of the Sources Sought response. Responses should be delivered via email to the addressee identified below. The email shall use the Subject: Sources Sought Response: Integrated Health Community, Health Management Software. Email: christopher.jennen@med.navy.mil 1.2 Purpose The IHCI is seeking software technology to enable beneficiaries to better manage their own health. Access may be provided to all Naval Medical Center San Diego (NMCSD) patients. The scope this project includes providing all installation, integration, maintenance, user and administrator assistance hotlines, and ongoing improvement to deliver on product capabilities, and all other items necessary to services in accordance industry standards, the current Joint Commission Manual, Occupational Safety and Health Agency, and all applicable local, State, and Federal laws, regulations, codes, and directives. 1.3 Background and Overview Founded in 2011, the IHCI is a multi-year initiative focused on building a comprehensive culture of health among military beneficiaries in San Diego County. The initiative seeks to move patients from healthcare to health through the seamless integration of military, virtual and community health services. It also targets various populations with a level of appropriate intensity needed to maintain a state of health and well-being. 2. Scope of This Sources Sought 2.1 Business Models 2.1.1 What is the typical business model for population health management consumer-facing technology? 2.1.2 How/when is technology generated data transferred to the Government? 2.1.3 How is software priced? 2.2 Technology Features Please describe your technology-enable population health management strategies to include, but not limited to: 2.2.1 Engaging consumer-facing dashboard 2.2.2 Customizable look and feel 2.2.3 Health assessments and ability to create or accommodate existing assessments 2.2.4 Individualized health plans based on interests and assessment results 2.2.5 Health status and behavior trackers 2.2.6 Accredited health education information modules 2.2.7 Gamification and rewards 2.2.8 Group challenges 2.2.9 Health campaigns 2.2.10 Mass messaging 2.2.11 Targeted messaging based on user health profile 2.2.12 Secure social networking, including private groups and communication 2.2.13 Role-based accounts 2.2.14 Reporting capabilities generated from system activity 2.2.15 Ability to integrate with existing internal health systems, such as RelayHealth 2.2.16 Ability to integrate a suite of health devices and applications 2.2.17 Rule-based alerts based on utilization-generated data 2.2.18 Multi-modal interfaces 2.2.19 Any other features or functionalities not listed here 2.3 Performance/Impact Measurements 2.3.1 What improved health outcomes should the IHCI expect to see? What are specific time frames to expect these outcomes, and what is the relationship of utilization metrics to typical outcomes? 2.3.2 What healthcare cost savings should the IHCI expect to see when employing the use of such technology to manage population health? What are the specific cost-savings relationships between utilization and healthcare costs? What are the associated time frames for ROI? 2.3.3 What behavior change should the IHCI expect to see when employing the use of such technology to manage population health? What are the causative relationships between utilization and behavior change? 2.3.4 What other impact on population health management should IHCI expect to see associated with your technology? 2.3.5 Are there any positive externalities aside from health measures associated with the use of your technology? 2.3.6 Please provide current client list. 2.3.7 Any white papers, impact reports, ROI history and/or other measured efficacy of system in achieving improved health outcomes and decreased health costs. 2.3.8 Please provide demonstration of ability to engage consumers in taking responsibility for their health management and outcomes, to include testimonials of other clients, users and administrators. 3. Other Information 3.1 Information Assurance: Please describe ability to work within Department of Defense (DoD)-mandated Information Assurance constraints with regard to data and network security. This includes measures to safeguard Protected Health Information/Personally Identifiable Information (PHI/PII) in compliance with the Health Insurance Portability and Accountability Act (HIPAA) requirements. 3.2 The IHCI intends to make available for review, information on its current health technology platforms, for the sake of scoping integration. What IHCI information/data would industry require in order propose for this effort? 3.3 The IHCI requests any industry partners to identify any existing contracts the IHCI may be able to leverage in bringing its requirement under contract. 3.4 What information do industry partners believe the IHCI should be considering in evaluating the capabilities of industry to employ technology-enabled population health management strategies throughout the IHCI and how should the IHCI ensure that it is continually optimized and modernized throughout the performance of the contract? In what format should this information be presented in order to allow industry to fully demonstrate their capability and allow the IHCI to conduct efficient evaluation? 4.
