- THIS OPPORTUNITY IS AVAILABLE ONLY TO SMALL BUSINESSES. The Lebanon VA Medical Center is soliciting quotes under Solicitation No. VA244-14-Q-0794 for the following: 'provide preventative maintenance and service to the Clever Brooks boilers and control panels three (3) times per contract year at the Lebanon VAMC located at 1700 South Lincoln Ave, Lebanon PA 17042. Contractor is to furnish labor and materials and perform work for the VA Medical Center in Lebanon, PA, in accordance with all specifications in the Statement of Work. The Solicitation documents will be available for download from the web https://www.fbo.gov on or about April 22, 2014. Potential offerors are responsible for monitoring this site for the release of the solicitation package and any other pertinent information. The Government will not provide paper copies of the solicitation. Offerors will be responsible for downloading their own copy of the solicitation package and subsequent amendments, if any. Telephone, written, facsimile, or e-mail requests for the solicitation will NOT be honored. It is essential that interested parties register on the Interested Vendor Listing at https://www.fbo.gov, 'Interested Vendor List'. Individual copies of the Interested Vendor List WILL NOT be prepared nor distributed by the Contracting Officer. The solicitation amendments issued to the RFQ will be posted to the website and bidders are advised it is their responsibility to obtain and acknowledge all amendments. Paper copies of amendments will NOT be individually mailed. All requests for information or questions regarding the solicitation must be submitted in writing to the Contracting Officer via mail or by email: carl.devitz@va.gov no later than Close of Business, April 30, 2014. No telephonic or fax requests will be honored. In accordance with FAR Part 36.204, the magnitude of this project is between $15,000 and $25,000. NAICS Code 811310 with a small business size standard of $7 million. The solicitation will close on April 30, 2014, at 4 pm EST. After the solicitation closes, an award will be made to the lowest responsive and responsible bidder. All interested contractors must ensure that their firms have the ability to provide the required labor, resources, insurance and licenses to perform the work. The government contemplates award of a Firm Fixed Price contract resulting from this solicitation. The government is also contemplating the use of two (2) option periods for this solicitation. Award of this contract will be for the base year. The maximum time for completion of the first service will be within 30 days of contract award. Offerors must complete their Representations and Certifications at the System for Award Management (SAM), www.sam.gov. If the offeror is a Small Disabled Veteran Owned Business, the offeror must be listed as verified in the vendor information page (VIP) at www.vetbiz.gov. Contractor must have a Dun & Bradstreet rating of sis (6) or lower.
