- AF 2755 Respirator - Annual Questionnaire Respirator - Initial Questionnaire SF 600 WG 2610 SF 78 Solicitation Document - Amendment 1 Solicitation Document The Government is seeking contractors that can provide Medical Physical examination services for the U.S. Air Force Reserve at Grissom ARB, IN. This procurement will conducted as a 100% small business set-aside with NAICS Code 621111 Offices of Physicians, with a small business size standard of $10.0M. All responsible offerors must be CCR registered to be considered for award. Potential offerors are responsible for monitoring the FBO site for the solicitation and other pertinent information and downloading their own copies. Contractor is to provide all labor, facilities, material, equipment, labs and necessary medical testing and supervision necessary to perform examinations for applicants/employees, to include but not limited to: Pre-employment physicals, Fitness-for-duty physicals, Occupational and occupational annual exams, Post-exposure evaluations, Employment termination physicals, Overseas employment physicals, and Vision and hearing exams as specified on Standard Form 78, Certificate of Medical Examination, or other designated forms.
