Description

This is a sources sought notice only and it is not a solicitation announcement. The purpose of this sources sought notice is to gain knowledge of qualified vendors that can provide the required services in Sioux Falls, South Dakota. The North American Industry Classification System Code (NAICS) is 624221 with size standard of $7.0 million. The VA Health Care System located at 2501 W 22nd Street in Sioux Falls, South Dakota, has a need to support Health Care for Homeless Veterans Contract Emergency Residential Services. It is understood that the type of veterans to be cared for under this contract will require care and treatment services over and above the level of room and board and will include both men and women Veterans in a residential setting. The number of beds currently required is nine (9) per day, but total beds needed may increase over the period of the contract. The contractor shall furnish each Veteran authorized care under this contract with the following basic services: (a) Residential Room and Board with individual rooms, (b) Ensures that Veterans have available food stores for three nutritionally adequate meals per day, (c) Provide Veterans with resource information on special dietary needs, (d) Provide quarterly technical performance report, (d) Establish participant record for each Veteran which includes required information, (e) Utilize HIMS for program participants, (f) Utilize internal tracking system, (g) Provide for physical security as required by VA inspections, (h) Provide Veterans with necessary supports to achieve access to services needed, (i) Provide on-site laundry facilities for residents to do their own laundry, (j) Case management services to assist in obtaining permanent housing, (k) Case management to assist obtaining employment for those who are able to be employed, (l) Collaboration with VA staff including but not limited to : admission, treatment and discharge, and (m) Therapeutic, Rehabilitative, and Recovery Services determined to be needed by the individual resident in a plan developed by the contractor with consultation by the Veteran and the VA case manager and/or other appropriate VA staff. Services which the contractor must be able to furnish include: (1) Structured group activities as appropriate - examples include group therapy, social skills training, (2) Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and (3) physical activities as appropriate. To respond to this sources sought notice, please complete the attached document titled "Sources Sought Response" and return per the included instructions. The closing date and time of this sources sought notice is April 24, 2014 at 4:00 PM ET. This is not a solicitation and in no way obligates the Government to award any contract. Additionally, a response to this sources sought notice is not a request to be added to a prospective bidders list or to receive a copy of a solicitation.
