- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Combined Synopsis Solicitation - Crane Rental and Crane Operator Response Date: 17 April 2014 at 1500 Hours, Mountain Standard Time (MST) Solicitation No: W9124R-14-T-0129 This is a Combined Synopsis/Solicitation (CSS) for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2005-72, Effective 30 Jan 2014 and Defense Federal Regulation Supplement (DFARS), current to DPN 20140331 (Effective 31 March 2014) Edition. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil. This solicitation is being issued as a Request for Quotations (RFQ) under Solicitation Number W9124R-14-T-0129 which will result in a single-award, firm fixed price contract. This action is being solicited as a 100% small business set aside. The North American Industry Classification System (NAICS) code is 238990, Crane rental with operator, with a size standard of $14,000,000.00 Dollars. Quotes will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis and tradeoffs are not permitted. Award will be made on an all or none basis. An offeror must quote on all items in this solicitation to be eligible for award. The United States Army Yuma Proving Ground (USAYPG), Yuma, Arizona is requesting quotes for the following: Contract Line Item Number (CLIN) 0001, Crane Rental and Crane Operator; Quantity: one (1); Unit of Issue: Jobs; (CLIN) 0002, Crane Rental and Crane Operator; Quantity: one (1); Unit of Issue: Jobs and CLIN 0003, Contract Manpower Reporting (CMR); Quantity: one (1): Unit of Issue: Jobs. Per attached Performance Work Statement, period of performance is from April 2014 - 24 April 2014 (2 Days) and 29 April 2014 - 30 April 2014 (2 Days). All quotes shall include transportation costs to USAYPG Yuma, Arizona 85365-9498 and be clearly marked with solicitation number W9124R-14-T-0129 and emailed to the Point of Contact (POC) listed below or sent by facsimile to 928-328-6534. Arizona vendors are to include Arizona Privilege Tax. Shipping and taxes (if applicable) shall be incorporated into your proposed price. Payment will be made through Wide Area Work Flow (WAWF). Delivery shall be made to USAYPG, AZ 85365-9498 with FOB Destination. All responding vendors must be actively registered with the System for Award Management (SAM) prior to award of contract. Information to register in SAM can be found at https://www.sam.gov/portal/public/SAM. All responding vendors must fill out the DFARS 252.209-7993 - Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law - Fiscal Year 2014 Appropriations attachment and sent back with your quote. This RFQ closes on 17 April 2014 at 1500 Hours MST. Responses to the RFQ shall be submitted by the closing date and time.
