- This notice FA7000-14-T-0030 issued by the 10th Contracting Squadron, United States Air Force Academy (USAFA) is provided to advise interested parties of the intent of the USAFA to enter into one or more Franchise Agreements for the provision of cable television services for all areas of the base in accordance with AFI 64-101, Multichannel video Programming Distribution (Broadcast Cable) dated 1 December 2005. This announcement constitutes the only notice, no other notice or solicitation will be issued. Specific information is being requested for respondents to be considered as stated below. This request for information incorporates clauses in effect through Federal Acquisition Circular (FAC) 2005-72 effective 30 Jan 14 and provided either in full text or by reference below. North American Industry Classification System Code (NAICS) 517220 and business size standard of 1,500 employees are being applied for this effort. This Franchise Agreement is being offered on an unrestricted basis. All interested responsible sources (per FAR 9.1) who indicate interest by responding by the date and time with the information requested below will receive consideration for to opportunity to enter into a Franchise Agreement with the USAFA. If interested in a Franchise Agreement with the USAFA for Cable Television Service, submit the following information on company letterhead to include: - Solicitation number - Contact name - Address - Telephone number of the offeror - Cage code - Duns number - Tax identification number - Size of business Information must be received no later than 3:00PM mountain Standard time (MST), 15 May 2014 at 10 CONS/LGCB, 8110 Industrial Dr, Suite 200, USAF Academy CO 80840-2315. Applicable Federal Acquisition Regulation (FAR) clauses incorporated by reference: 52.249-4 Termination for Convenience of the Government (Services)(Short Form)(APR 1984) 52.249-8 Default (APR 1984) Applicable FAR clauses in full text: 52.242-2 Clauses incorporated by reference(FEB 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. (end of clause) 52.228-5 Insurance - Work on a government Installation (JAN 1997) (a) The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required in the Schedule or elsewhere in the contract. (b) Before commencing work under this contract, the Contractor shall notify the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government's interest shall not be effective -- (1) For such period as the laws of the State in which this contract is to be performed prescribe; or (2) Until 30 days after the insurer or the Contractor gives written notice to the Contracting Officer, whichever period is longer. (c) The Contractor shall insert the substance of this clause, including this paragraph (c), in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the Schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors' proofs of required insurance, and shall make copies available to the Contracting Officer upon request. (End of Clause) NOTE: Levels of insurance to be maintained shall be at a minimum the same as those identified in FAR 28.307-2 (a)(b) and (c).
