- Statement of Work The contractor shall provide preventive maintenance and repair the equipment in accordance with the manufacturer's specifications. The contractor shall provide service which may consist of calibration, cleaning, oiling, adjusting, replacing parts, and maintaining the equipment, including all intervening calls necessary between regular services and calibrations. All required parts shall be available. The contractor shall repair any components of the dialysis equipment that are not working properly, at no additional charge, provided replacement is due to normal wear and tear. Scope of Work Contractor shall provide support for the following models and serial numbers: ModelEESerial Number Millennium40022117626 Zyza3252717964 Zyza3252617965 23G294561224205 WROH558321300350 WROH558331300353 WROH558341301187 WROH558351301186 WROH558361301188 1. SERVICES: Contractor shall furnish the following support and maintenance services for the contract period of April 1, 2014 through September 30, 2014. Services for 23G: Monthly: Contractor shall complete system performance review, disinfect membranes, clean membranes, disinfect loop, performance and replacement of RO prefilter, and provide a 5M 20" filter. Quarterly: Contractor shall complete performance and replacement of pyrogen and vent filter and provide 0.05M 20" filter, 0.2M 10" filter, and 10 bags of salt. Services for Millennium and Zyzas: Monthly: Contractor shall complete portable performance review, performance and replacement of RO prefilter, and provide a 5M 10" filter. Quarterly: Contractor shall provide and connect rental carbon tanks that are 8x18 and 0.50 cubic feet. Carbon tanks must be installed and the old tanks removed. Services for WROHs: Monthly: Contractor shall provide portable performance review, clean portables, performance and replacement of RO prefilter, provide 5M, 10" filter, and initiate heat disinfection. Quarterly: Contractor shall provide PTG carbon block and exchange the carbon block filter PTG. Portable performance review includes a full inspection of the portable dialysis water system. System performance review includes a full inspection of dialysis water system. Documentation: After which time the services have been completed; all service work performed, results, readings, and data shall be reported. 2. PARTS, LABOR, and TRAVEL: All parts, labor, and travel to complete the services listed in Section 1 shall be provided as part of the service contract. 3. EXCLUSIONS: Any additional services outside the scope of this contract shall be billed separately based on time and materials. 4. COMPETENCY OF PERSONAL SERVIVING EQUIPMENT Fully Qualified" is based upon training and on experience in the field. For training, the FSE(s) shall have successfully completed a formalized training program, for the equipment identified items in this document. For field experience, the FSE(s) shall have a minimum of two years of experience, with respect to scheduled and unscheduled preventive and remedial maintenance on equipment identified in this document. The FSE(s) shall be authorized by the contractor to perform the maintenance services as required. All work shall be performed by "Fully Qualified" competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the contractor services upon the request of the G.V. (Sonny) Montgomery VA Medical Center. 5. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS. The contractor's FSE shall wear visible identification at all times while on the premises of the VAMC. It is the responsibility of the contractor to park in the appropriate designated parking areas. The VAOPC shall not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Cellular phones and two-way radios are not to be used within six feet of any medical equipment. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court.
