- I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with no additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number F14PS00406 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This is a small business set-aside Request for Quote (RFQ). II. This procurement is for Line item 010: 33C Limestone Class A, Grade D per TDOT Gravel specification; for delivery and tailgate spread at Haul Road Extension located near the Duck River Unit of the Tennessee NWR. Line item 020: Dirt Fill purchase, delivery with tailgate spread and spot dump at low lying spots; Location Haul Road extension. III. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items and an award will be made in accordance with FAR Part 13, Simplified Acquisition Procedures. This action is being solicited as a small business set-aside and only qualified offerors may submit a quote. The NAICS Code associated with this requisition is 212321 with a small business size standard of 500 employees. Buyer intends to issue award using a fixed priced purchase order or delivery order. Bids from Sellers unable to accept purchase orders or delivery orders will not be considered for award. IV. Offerors are cautioned that a warranted Contracting Officer is the only authorized Government Representative to make any contractual changes once an award is made. No oral statement of any person whomsoever shall, in any manner or degree modify or otherwise affect the terms of this Purchase Order. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the Purchase Order price to cover any increase in costs incurred as a result thereof. The CO for this order is James_Promise@fws.gov. V. Government Representative (GR): Authorized Government Point of Contact for this order is Heather Lambert at heather_lambert@fws.gov. She does not have the authority to make any changes in the purchase order. (a.) RFQ due date: Friday, April 22, 2014. (b.) RFQ due time: 11:00 A.M.; Local Eastern Time. (c.)The anticipated award date is Friday, April 29, 2014 or earlier. Please do not request award status prior to this date. VI. Description: (1.) Line Item 0010: 33c Crushed Limestone; Class A, Grade D per TDOT; Quantity: 3500 Tons The work entails furnishing all necessary management, supervision, labor, materials, and equipment to supply 3500-tons of 33c, crushed limestone for delivery and spread with a minimum of 250 tons delivered per day. This will be under the direct supervision of Refuge Staff. The contractor shall ensure total for all amounts supplied does not exceed the amount noted in the contract line items. (2.) Line Item 0020: Dirt Fill; Quantity: 2200 cubic yards. The work entails furnishing all necessary management, supervision, labor, materials, and equipment to supply 2200 cubic yards of dirt. Dirt fill is to be delivered and tail gate spread (and spot dumped at low areas as directed) at Haul Extension located near the Duck River Unit of the Tennessee NWR under the direction of Refuge staff upon delivery. The contractor shall ensure total for all amounts supplied does not exceed the amount noted in the contract line items. VII. Delivery Requirements: Delivery will be near the Duck River Unit of the Tennessee NWR. Coordinate with Refuge staff about delivery quantities and exact locations. Due to the remote location, the contractor will be expected to assign more than one truck at a time to deliver on the site. The location is too remote to receive one load a day. A minimum of 10 trucks would be expected daily. There are county road weight limit restrictions coming into this site the vendor needs to be aware of. The vendor would need to verify with the county road commissioner if a bound is required to haul on this road. Please reference attached Roadway/Gravel Surface Material sheet, attached map for detail of exact locations, and/or contact the technical point of contact (David Zabriskle, 931-535-2465; david_zabriskle@fws.gov) for exact location specification. Once delivery begins, the Contractor shall deliver at a rate of not less than 10 truckloads per day during regular working hours. Hours are 7:00 A.M. to 3:30 P.M. The delivery will accompany a tailgate spread or spot dump with amount piled and thickness to be determined by the site manager at the designated location. Identified delivery location will be at designated through site manager from Duck River Unit of TN NWR. All deliveries will occur within an active jobsite or staging area that will accommodate highway compliant vehicles, required delivery trucks, etc. Drivers shall furnish certified scale tickets to FWS/government personnel upon delivery of each load to the delivery site. Required commencement date for delivery is on or about June 2, 2014 or sooner. Last day for delivery is September 30, 2014. NOTE: It is the Contractors responsibility to visit the site prior to bidding and know existing delivery and dumping conditions. VIII. Local conditions (bridge restrictions, weather conditions, water levels, road flooding, etc.) that may affect delivery: None. Contractor shall coordinate deliveries (including hours of delivery) with GR 48 hours in advance. IX. System for Award Management (SAM) Registration - A prospective awardee shall be registered in the SAM database prior to award. Information on registration may be obtained via the Internet at: https://www.sam.gov/portal/public/SAM/#1 or by calling 866-606-8220. Failure to maintain a current/active account so will render your offer non-responsive. X. RFQ Due Date: Telephone quotes will NOT be accepted. All quotes must be received at this office no later than April 22, 2014, 11:00 AM Local Eastern Time. ONLY EMAILED QUOTES WILL BE ACCEPTED. Email to constance_fields@fws.gov. Attention to: Constance Fields. Please reference/subject: solicitation number and Gravel Solicitation Duck Unit; Tennessee NWR. Quotes should be attached document that is either Microsoft Word 97 compatible or PDF. The anticipated award date is Friday, April 29, 2014 or earlier. All questions must be submitted no later than April 16, 2014 by 11:00 AM local eastern standard time. XIV. All interested Contractors shall provide a quote as follows: (1) SF 1449 is acceptable for submitting quote. Parties responding to this solicitation may also submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.), but at the minimum include the following information: (2) Line Item 0010: 3500 tons of 33c Crushed Limestone Price per Ton $________; Total Price $_______; Line Item 0020: 2200 cubic yards of dirt fill price per cubic yard $________; Total Price $_______; Line 0010 and Line 0020; Grand Total___________________________. (3) Company's name, point of contact, address, phone number, fax number, email address, solicitation number, item Description/Specification (Descriptive literature if applicable), total price (itemize price final total price),and approximate daily delivery schedule ( include number of trucks, amount to be delivered, and total number of days to complete requirement). Price(s) should be for FOB Destination. (4) A list of three (3) most recent past performance references for a similar purchase including name/point of contact, address, phone number, and email address; (5) Statement confirming registration in SAM (www.sam.gov). Statement confirming Representation and Certification completion within SAM. Provide account expiration date, cage code, DUNs number, and taxpayer ID number; (6) Statement confirming that you will accept electronic payment through IPP. (www.ipp.gov) NOTE: Any quotes not received by the due date and time specified in this synopsis/solicitation will be rejected. Additionally, any incomplete quote not containing the required information as stated in this solicitation may also be rejected. NOTE: The final quoted price(s), both total and individual, MUST include all applicable fees, gratuities, commissions, service charges, etc. THE FEDERAL GOVERNMENT IS TAX EXEMPT. XII.
