- Compounded Pharmaceuticals This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is N00259-14-T-0138. It is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This is a small business set aside action. The North American Industry Classification System (NAICS) Code for this acquisition is 325412. The size standard is 750 employees. The contractor shall provide all services, materials, and equipment necessary for compounding and deliveries of Inpatient Narcotic and Anesthesia Pharmaceutical items for the Main Pharmacy, Naval Medical Center San Diego, San Diego, CA. LIABILITY: The contractor shall not be liable for any loss, damage, or delay due to any cause beyond his reasonable control including but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of God. CONTRACTOR CHECK-IN/CHECK-OUT: The contractor shall coordinate with the Main Pharmacy and Materiel Management to get access inside Naval Medical Center San Diego Base and the Main Pharmacy. Labor: All compensation for labor is included in the contract price. Parts and materials: All compensation for parts and materials is included in contract price. Parts/Supplies Quality: Contractor required providing at least yearly, pharmacy and compounding licenses and quality assurance data for 1-3 lots of each medication ordered. Contractor must allow NMCSD Pharmacy personnel to inspect compounding facility within 90 days of contract award and at least yearly. Contractor must pass inspection by NMCSD Pharmacy personnel for continued service. Basis of inspection will be the American Society of Health Systems Pharmacist's Outsourcing Sterile Products Contractor Assessment Tool. Parts and Supplies provided under this contract shall be guaranteed to be equal in all respects, including performance, interchangeability, durability, and quality. Products shall meet American Society for Testing and Materials (ASTM Background Colors for User-applied Syringe Drug Labels and shall include TALL MAN lettering and expiration date. For all CII pharmaceuticals appropriate paperwork will be FedEx Overnight with vendor Billable Stamp to vendor. DELIVERIES: Monday through Thursday orders are requested to be delivered in the morning next day or no later than forty-eight hours from date and time order was placed. Friday orders will be delivered no later than Tuesday morning, and NMCSD will not be charged for any shipping charges on any delivery within the requirement of this contract. Delivery services for NMCSD to be provided in compliance with United States Pharmacopeia (USP) Chapter 797 and Good Compounding Practices (USP 1075).
