- 1.1 Requirement: The New Hampshire National Guard has a requirement to contract for care coordination and referral services to members of the US Armed Forces as defined in 10 USC 10(a) (4.) and (c) and their families within the state of New Hampshire. 1.2 Purpose: The goal of the program is to enhance the retention, readiness and reintegration of service members and their families. 1.3 Audience: The beneficiaries of the Care Coordination Program are all National Guardsmen, Reservists, and Active Duty Service Members and their families who are geographically separated from military installations and live or work in the State of New Hampshire. For data collection and historical reporting, this population is divided into service members and families who are within the timeframe of the 4-stage deployment cycle as defined by Public Law 110-181 and those who are outside this period. a. Within Deployment Cycle population: Recognizing that each service and component of the military has a unique deployment cycle, for the purposes of this contract the deployment cycle is defined as the time period from when a Service Member and family are alerted of an upcoming deployment, throughout that deployment, demobilization and up to 180 days post-deployment. b. Out of Cycle: Out-of-Cycle refers to all post-9/11 Service Members and their families who are not In-Cycle as defined above, but who have unmet needs for the services described in this Performance Work Statement. 1.4 Objectives: The Care Coordination Program will meet its purpose through two primary objectives: Care Coordination and Outreach. a. Care Coordination: Care Coordination is personalized and substantive services provided to Service Members and their families. There are two types of Care Coordination: Direct and Indirect. The priority is always Indirect, though Direct Care is authorized during emergency situations or when significant barriers to entry to outside agencies exist. i. Indirect Care: This includes intake, assessment of needs, development of Care Plans, coordinating care, and follow-up. Care Coordination involves identifying unmet needs, and developing plans to meet those needs. Care Coordination is always intended to supplement, not supplant, services available from federal, state and community-based organizations; Indirect Care is broadly defined as Care Coordination and referrals that maximize participation inside of those federal, state, and community-based organizations. ii. Direct Care: Direct care by a licensed clinical social worker during emergency situations or when significant barriers to entry to outside agencies exist. Direct Care can be considered those services which would otherwise be provided by outside agencies at the federal, state, and local level, but barriers such as timeliness and availability affect access. b. Outreach: Outreach is divided into Community Capacity Building and Outreach to military units, Service Members and their families. i. Community Capacity Building: The goal is to develop relationships with local providers in the state who can provide needed services to Service Members and families, and to leverage these local providers in the execution of the Care Plans. These activities involve regular attendance at various committees, education and outreach to providers, and routine liaison with providers throughout the state. These activities both create and coordinate military/civilian relationships, thus creating a stronger safety net for our military. ii. Outreach to military units, Service Members, and their families: The goal of these activities is to increase awareness of the Care Coordination Program and resources available to Members of the Armed Forces and their families. Activities include but are not limited to outreach education at unit drills, family days, organization days, or at other events when invited by a unit; outreach at veteran fairs and events throughout the state; and will include a plan to proactively market to families and active duty individuals geographically dispersed throughout the state. This is a Sources Sought market survey for written information only. This is not a solicitation announcement for proposals, and no contract will be awarded from this announcement. All information shall be provided free of charge to the Government. The purpose of this Sources Sought announcement is to identify sources capable of providing support for this effort. The information received will be used to determine if the whole requirement, or a portion of the requirement, can be set aside for small business, 8(a) business development participants, HUBZone small business concerns, service-disabled veteran-owned small business concerns, economically disadvantaged women-owned small business concerns or women-owned small business concerns eligible under the Women-Owned Small Business Program. Responses from small businesses are highly encouraged. The Government reserves the right to consider a small business set-aside or 8(A) set-aside based on responses submitted hereto. NAICS Code 624190 applies. Small Business Size Standard is $10,000,000. The NH-USPFO, Purchasing and Contracting Office, Pease ANGB, Newington New Hampshire has a requirement for care coordination and referral services to members of the US Armed Forces as defined in 10 USC 10(a) (4.) and (c) and their families within the state of New Hampshire. The general level of support representing the core requirement consists of the following: The specific level of support will be detailed in future Performance Work Statement (PWS). Interested small businesses concerns should submit capability statements reflecting their companies' capabilities to provide the effort as described by the core requirement. Note the core requirement is subject to change. Interested companies shall include the following information in their responses: (1) CAGE CODE (2) Description of their capability of performing the requirements in this Source Sought; (3) Name and address of firm; (4) Size of business including average annual revenue for past three years, number of employees, and type of small business (8 (a), Women-Owned, HUB-Zone, Veteran- Owned, Service-Disabled-Veteran-Owned Business); (5) Ownership including Country of Ownership and DUNS Number; (6) Number of years in business; (7) Two points of contact including: name, title, phone and e-mail address. Information provided will be treated as Business Sensitive and will not be shared outside of Government activities and agencies without the permission of the provider. NH-USPFO PCO may request further information regarding the capabilities of respondents to meet the requirements and may request a presentation and/or site visit as deemed necessary. In the event a solicitation is developed, it will be assigned a formal Request for Proposal (RFP) number and the announcement will be published on FBO.
