- Description
- The Department of Veterans Affairs, Veterans Health Administration, VA White River Junction Healthcare System, has a requirement for a technological tool that contains software to automatically track and monitor the patient discharge and referral process from an acute care Medical Center. The successful contractor's solution shall provide a secure web site that is accessible to VAWRJ staff. The web site will allow VAWRJ staff to input referrals to the network's participating facilities. The web site will facilitate exchange of information between the Medical Center and the network's community facilities and expedite discharge of patients from the acute care setting into the community facilities. Attached is a Request for Quotation (RFQ) that will result in a fixed price contract. A base of one year with four (4) 1-year option periods is anticipated. This solicitation is 100% set-aside for small business under the NAICS code of 511210 (small business size standard $35.5M). Interested parties not currently showing this NAICS under their System for Award Management (www.sam.gov) registration are encouraged to update their registration. Successful offeror shall have their SAM registration updated to reflect this NAICS prior to any award being finalized. Interested parties shall respond via email to the Contracting Officer no later than April 17, 2014 no later than 1:00pm Eastern Standard Time. No phone calls will be accepted; communication shall be via email only. Interested parties having questions may submit questions to the CO via email. Questions submitted after April 8, 2014, Noon Eastern Standard Time will not be addressed. A single amendment to the solicitation will be issued to ensure all parties have the same information. Inquiries/information received after the established deadline will not be considered. The Government does not intend to pay for information solicited.
