- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation, Subpart 12.6, and 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested see additional information for a copy of the solicitation issued. This requirement is 100 percent set-aside to small business. The NAICS code for the acquisition is 562211. The size standard is $35.5M. The ARNG Medical Command, located at Camp Joseph T. Robinson, North Little Rock, AR intends to procure regulated medical waste services for 2 facilities at Camp Robinson in North Little Rock, AR and 1 facility at Fort Chaffee in Fort Smith, AR. Please see attachments (link below) for Solicitation document, Performance Work Statement and applicable SCA Wage Determination. Interested parties capable of meeting this requirement MUST submit a quote to include their CAGE code, DUNS and any other pertinent information. Oral quotes will not be accepted. To be eligible to receive an award resulting from this RFQ/solicitation, the contractor must be registered in SAM (System for Award Management www.sam.gov) as of the date of quote submission. Provision 52.212-2, Evaluation-Commercial Items applies to this acquisition. For the purpose of this provision the following evaluation factors will be used. 1) Technical capability: Vendor will submit copies of license or certificates required by Federal and State agencies to transport and handle medical waste. Vendor will submit information that details the methods and sources of training required by the performance work statement. 2) Past Performance: Vendor will submit names, phone number and email addresses of 3 customers who they have provided the same or similar services too. 3) Price: Vendor will submit a quotation for services that includes cost of labor, equipment, installation, transportation and any shipping or handling charges. Provision 52.212-1, Instructions to Offerors -- Commercial Items applies to this acquisition. Submission of offers. Submit signed and dated quotes/offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show -- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price and any discount terms; (7) quote mark Remit to quote mark address, if different than mailing address; (8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and (11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. 12) Submit Technical information, Past Performance references with price information no later than the date and time required in block 8 of the SF 1449 for submission of proposal/quote to be considered for award. All Information shall be submitted to the POC listed in Block 9, or by email to thomas.w.yerkey.civ@mail.mil no later than the date/time also in Block 8 of the SF 1449. NOTICE OF PRE-QUOTE SITE VISIT An onsite pre-quote site visit can be conducted by contacting the government points of contact listed in the performance work statement or the point of contact below. The purpose of the site visit is to allowing interested parties to see the existing facilities and locations for work. All questions regarding this solicitation must be submitted by email to Thomas Yerkey Thomas.w.yerkey.civ@mail.mil. Bidders or offerors will not be reimbursed for any cost to attend a site visit or provide a quote or offer. Any information provided at the site visit shall not qualify the terms and conditions of the solicitation and specification. Terms of the solicitation and specifications remain unchanged unless the solicitation is amended in writing.
