Attachment 2: Request for Sole Source Memo Format DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 11, VA Ann Arbor Medical Center 506-14-2-522-0267. 2.Nature and/or Description of the Action Being Processed: Installation and relocation of Access Control Point Security Devices for the existing Hirsch digital security system at the Radiology MRI area. The contractor will utilize existing fully functional customer provided electrical door hardware and power supply. The contractor will utilize existing access control input on the M8N2 controller that is located in room D321A. 3.Description of Supplies/Services Required to Meet the Agency's Needs: $5,565.38. Security Corporation, 22325 Roethel Dr, Novi, MI 48375. Period of Performance 3/17/14 to 3/21/14. The contractor will Install Access Control Point to door #5 and the removal and relocation of security devices on doors, #1, #2, #3, #4 located on the third floor Radiology MRI Area. The Install will include all labor to install cabling and cabling supports, system programming, testing and final adjustments. 4.Statutory Authority Permitting Other than Full and Open Competition: ( x ) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per FAR 6.302-1; ( ) (2) Unusual and Compelling Urgency per FAR 6.302-2; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability or Expert Services per FAR 6.302-3; ( ) (4) International Agreement per FAR 6.302-4 ( ) (5) Authorized or Required by Statute FAR 6.302-5; ( ) (6) National Security per FAR 6.302-6; ( ) (7) Public Interest per FAR 6.302-7; 5.Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): This vendor currently has a maintenance contract, VA251-14-C-0038 base thru 9/30/2017 for maintenance of the Hirsch Security Access Equipment at the VAAAHS. By having this vendor install the equipment, this equipment will be covered by a maintenance contract through 2017. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: FSS and the internet were searched. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: $5, 565.38.
