- This is a questionnaire being conducted in conjunction with U.S. Air Force (USAF) Draft Request For Proposal FA8056-14-R-0004. Listed below please find the link to the questionnaire: https://www.surveymonkey.com/s/FA8056-14-R-0004 The purpose of this questionnaire is to assist the USAF in assessing the ability of Fire Emergency Service (FES) Personal Protective Equipment (PPE) suppliers to successfully meet and/or manufacture/distribute products under new United States Air Force (USAF) requirements. These scenarios are provided for information purposes and does not imply a government obligation or commitment to implement. The questionnaire consists of 19 pages, 46 questions in total and may require up to 2 hours to complete. Questions with an asterisk (*) are required. Please complete this survey by COB Thursday, 10-April-2014. To advance the survey, click the "Next" button. To go back a page, click the "Prev" button. Do not use your internet browser icons to navigate the survey. If necessary, you may exit the questionnaire while in process and resume on the same page at a later time.
