THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, Network Contracting Office-8, in support of James A. Haley Veterans Hospital, is performing market research to compile data regarding: (1) The availability and capability of qualified small business concerns; (2) Identify if there are sufficient number of qualified Service-Disabled Veteran-Owned, Veteran Owned, Small Business, HUB Zone 8(a), Women-Owned, or Small Disadvantaged Business concerns relative to the subject North American Industry Classification System (NAICS) code listed below. Responses to the information requested will assist the Government in determining the appropriate procurement method, including any set-aside decision. Respondents should clearly describe their capabilities. The NAICS code assigned is: 339112 Surgical and Medical Instrument Manufacturing. The size standard is 500 employees. Interested individuals shall provide a capability statement, to include your complete address, phone & email address of contact. All vendors shall be registered in System Award Management (SAM) to be considered for any contract award, and proof of certified Vet Biz registry (if applicable) is required. Note: Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Proposal. It is the intent of James A. Haley Veterans Hospital to use the information gathered for market research purposes only. If a formal solicitation is released, it will be posted via the Federal Business Opportunity (http://www.fbo.gov) DESCRIPTION OF THE REQUIREMENT: (1) CARTO 3 Cardiac Mapping System. Cardiac mapping is performed with catheters that are introduced percutaneously into the heart chambers and sequentially record the endocardial electrograms with the purpose of correlating local electrogram to cardiac anatomy. The CARTO ® 3 System includes:. Includes (i) baseline software license, (ii) location pad with foot pedal, (iii) grounding cable, (iv) optic cable, (v) power supply unit, (vi) patient interface unit, (vii) pin box (2), (viii) system cables, (ix) patch unit, (x) Windows Workstation Computer with two 24" high resolution flat screen repeater monitors, (xi) user's manual, (xii) color inkjet printer, (xiii) and physician and staff training. It includes a Siemens BWI Communication Upgrade for Sequoia or X300 ultrasound systems consistent with systems used in the Tampa VA EP lab. Also, it includes a VISITAG Module, a modular software package to be used with the CARTO ® 3 System. Software allows user to evaluate ablation points based on a variety of parameters, to consistently tag and evaluate ablation points. The PASO Module is a modular software packate to be used with the Carto 3 system to automate pace papping for ventricular tachycardia cases. The Clinical Module Package for CARTO ® 3 System Includes: The CARTOMERGE ® Image Integration Software Module, a modular software package to be used with the CARTO ® 3 System, that allows for importation of pre-acquired MRI and CT images direct from scanners or from radiology workstations, provides a semi-automated image segmentation package and allows for catheter icon registration to pre-acquired MRI and CT images. On-site training for technicians included. The CARTOSOUND ® Image Integration Software Module is a modular software package to be used with the CARTO ® 3 System, that allows for the integration of real time Intracardiac echo in an integrated and a side by side manner on the CARTO ® 3 System screen. This CARTOSOUND ® software is compatible with the Biosense Webster SOUNDSTAR ® 10F and GE and Siemens 10F and 8F ACUNAV Catheters. On-site training for technicians included. In addition, a Complex Fractional Atrial Electrograms (CFAE) Software Module License to be used with the CARTO ® 3 System is included. This enables the automatic detection of CFAE sites by analyzing individual signals from endocardial locations. The analysis is based on a series of advanced user defined parameters applied to the acquired signals. The analysis output provides a consistent method to quantify and characterize complex fractionated signals. This includes one executable copy of the software and one copy of the software user documentation per license, and on-site training for technicians. This quote includes the CARTO ® 3 Premium Service Upgrade for One Year which features "Parts, Labor and associated travel to service the CARTO ® 3 System - By local Field Service Engineer. Emergency / immediate support if required may be performed by non-local Field Service Engineer "Telephone Support "Priority On Site Support - for System failures that cannot be resolved through Telephone Support "Annual Preventative Maintenance "Two System relocations per Agreement Term - (Subject to BWI approval.) "Standard Wear and Tear (Subject to the terms and conditions.) "Accidental Damage The Non-Sterilized Cables to support CARTO ® 3 System - Includes Optic Cables, PIU Power Cables, Body Surface Cables, Pacing Cables, Video Cables, Parallel Cables, and IC Output Cables are included in this package.
