Description

The Department of Veteran Affairs, Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209 has a requirement for a Fabricate Counter Top. The Government expects to award to a single contractor a contract which shall be Firm Fixed Price with a firm quantity and no option years. This requirement is being set-aside for Service Disabled Veteran Owned Small Business participation only. NAICS code for this requirement is 337110 with a Service Disabled Veteran Owned Small Business size of 500 employees. Only offers from those firms regularly established in the business of providing this type of equipment will be considered. Subcontracting will not be allowed. Color to be determined from "C" series or top grade products. More details are on the pages that accompany this order including edges and splashes. Total square footage is approximately 163 square feet. Vendor will need to do a site visit to make field measurements once the area is ready for product. Color is not identified since each manufacturer had their own Color selection will be made once the contract has been awarded. Quantities are estimated since the area is still under construction. PERFORMANCE BASED WORK STATEMENT Fabricate, provide and install solid surface countertop 1. GENERAL: The Contractor shall provide all labor, equipment, materials and supervision necessary to provide and install fabricated solid surface countertop as shown on the sketches attached to the request. Pricing on the bids shall be separated into materials and labor costs and identified separately on the bid. Veterans Health Administration, William Jennings Bryan Dorn, Medical Center (WJBDMC) 6439 Garners Ferry Road, Columbia, South Carolina 29209. 2. BACKGROUND: The WJBDMC is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 56,116 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rockhill, Sumter, and Spartanburg, South Carolina. 3. PERIOD OF PERFORMANCE: Working normal business hours, on-site installation shall be completed within Five days of start (not including production time). The product and the installation will be warranted for 1 year after acceptance. 4. PLACE OF PERFORMANCE: New travel pay office, building 100 First Floor. 5. DAYS AND HOURS OF OPERATION OF THE FACILITY: Monday - Friday, 8:00 a.m. to 4:30 p.m. excluding federal holidays. 6. EQUIPMENT DESCRIPTION: Manufacture and installation of solid surface countertop. A sketch with notes has been included in the package. 7. PERFORMANCE REQUIREMENTS: a.Vendor will follow all OSHA requirements during installation. b.Vendor will clean up daily before leaving the worksite. c.Smoking is not permitted in the building or near entrances. d.On-site fabrication must be minimal. e.Vendor arranges a work schedule with us so we can make the appropriate notifications with a minimum of two working days in advance of arrival. f.Tools and equipment will not be left un-attended or left overnight. g.Vendor will maintain the barricaded construction area. h.Area will be left in "as good or better" condition than found. i.Packaging and shipping materials will be disposed of off-site by the vendor. No means of disposal or recycling is provided by VA. 8. CONTRACT PERFORMANCE MONITORING: The government reserves the right to monitor the installation in accordance with Performance Based Matrix. 9. INVOICES: Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer's Technical Representative (COTR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714.
