Description

THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. No solicitation will be issued. This is a notice of intent to award a sole source contract in accordance to FAR 6.302-1(a)(2), authorized or required by statue. This is a one-time procurement. The VISN 10 Network Contracting Office intends to award a sole source procurement with Bayer Healthcare. to purchase: Services to be Provided: A Preventative maintenance on Angiojet Ultra Thrombectomy pump used in the IR suite in Radiology by field service engineer from Bayer Healthcare. Bayer HealthCare Medical Care 100 Global View Drive. Warrendale, PA 15086. The contact engineer is Cliff McIntosh. 1-800-633-7231 (ext. 7332). Unit is located in Radiology IR room C216E located at VA Medical Center Cincinnati Ohio 45220. The serial number of pump is U2870. The model number is 5000AM/D. EE4713. This is a one time pm of this unit. There is no service agreement contract on this unit. The PM includes the following: "Verification and re-calibration per factory specifications "Software updates "Certified replacement parts "Complete inspection and safety testing "90-day warranty protection. "Bayer HealthCare Services available 8 AM - 5 PM M/F Services All service provided by the contractor shall meet the equipment manufacturer's original performance specifications. Contractor Service Report Contactor shall provide a written or electronic service report upon completion of any service activity. Minimum information included shall be date and time, service personnel name(s), problem description, corrective action, parts used, and hours on-site. The procurement will be for the VAMC Cincinnati, Ohio. The estimated amounted is $3,638.00. The Government believes that Bayer Healthcare. is the only company that can satisfy the agency requirements. Therefore, the Government intends to proceed with sole source procurement for the required equipment. To the best of our knowledge Bayer Healthcar is the only source that provides the above indicated services. The North American Industry Classification System (NAICS) code for this acquisition is 325413 and the size standard is 500 employees. This is not a request for proposals; however, an interested party may e-mail a written capability statement for these required items, which shall not exceed two (2) pages in length. Interested parties may submit their capability statement, which may or may not be considered by the agency via email to Jeffrey.black2@va.gov no later than 3:00 PM EDT on April 2, 2014. If no responses are received, the Cincinnati VA Medical Center will proceed with sole source procurement with Bayer Healthcare..
