Description

This is a Sources Sought (SS) in accordance with FAR 15.201(e). The government does not intend to award a contract on the basis of this request, as such, neither unsolicited proposals nor offers of any kind will be considered in response to this SS and there will be no contract award resulting from responses to this notice, nor reimbursement for any costs associated with providing information in response to this request. The purpose of this SS is to gain knowledge of potentially qualified manufacturers, producers, distributors and their size classifications for providing Reusable and Disposable Slide Sheets. The result of this market research will contribute to determining the method of procurement. Based on the information gathered from this SS, a determination by the contracting officer will be made to issue a solicitation and the applicable North American Industry Classification System (NAICS) code. The Department of Veteran Affairs, Strategic Acquisition Center (SAC) is issuing this SS as a means of conducting market research to identify qualified contractors with the capability and resources to meet the Veteran Health Administration's (VHA) requirement for Reusable and Disposable Slide Sheets, to all VHA within and throughout the United States, Alaska, Hawaii, Puerto Rico, Philippines, and Guam. DESCRIPTION OF WORK: Provide delivery of Reusable and Disposable Slide Sheets, to all VHA facilities in the United States, Alaska, Hawaii, Puerto Rico, Philippines, and Guam. EQUIPMENT IDENTIFICATION/SERVICE: Each vendor must be able to comply with the Minimum Technical Requirements (MTR) for the Reusable and Disposable Slide Sheets listed. The VA is interested in finding out if interested vendors can provide and/or manufacture the following Reusable and Disposable Slide Sheets: a.Reusable Slide Sheets: Intended Use - Reusable Slide Sheets are for the patient care management team to use in hospital for safe patient handling during inpatient care. b.Disposable Slide Sheets: Intended Use - Disposable Slide Sheets are to be used by the patient care management team for patient handling to reduce injuries related to lifting, repositioning and transferring patients. If your organization/company has the potential capability to provide the, Reusable and Disposable Slide Sheets as a distributor or manufacturer, please provide the following information: a.Please use attached spreadsheet to provide: Company Name and Capability Statement, Point of Contact address and e-mail, website address, DUNS number, telephone number, business size (ie. Large, small business, service disabled veteran owned, veteran owned, woman owned, disadvantaged, HubZone, or 8(a)) as applicable for NAICS Code, 339113 SIN Code 65 IIA/A-76. If your company feels that this requirement can be fulfilled under a different NAICS Code or SIN Code, please provide the suggested NAICS Code or SIN Code and a rationale for the recommendation. Please provide sizes your company can supply, whether or not has a handle, the weight limit, and the schedule number, if there is one, using attached spreadsheet. b.Manufacturer or Distributor: If you are a small business manufacturer/distributor your company must manufacture/produce or subcontract with another small business manufacture for the Reusable and Disposable Slide Sheets in accordance with the MTRs outlined above.
