Description

The Department of Veterans Affairs intends to negotiate a noncompetitive award to Carrot Medical, LLC, 22122 20th Avenue, SE Suite H-166, Bothell, WA 98021, a Small Business (SB), for Computer Training for the VA Palo Alto Health Care System, 3801 Miranda, Avenue, Palo Alto, CA 94304. This procurement is a sole source award under FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements. Sole source justification has been prepared in support of this acquisition. Firms believing they can provide these services must provide written notification to the Contracting Officer with supporting evidence to that effect in sufficient detail to demonstrate the contractor's ability to meet the stated requirements. This notice is NOT a request for competitive proposals. All responses to this notice are solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Responses must be emailed to Phyllis.Charles@va.gov and received by March 17, 2014, 4:00PM PST. Information should include Dun and Bradstreet Number (DUNS), Tax Identification Number (TIN), business size in accordance with NAICS code is 811219 and the small business size standard is $19.0 million. Scope of Work: The contractor shall furnish all labor, personnel, equipment, software, materials, loaner equipment, transportation and resources necessary to provide repair, replace and configure for two (2) Government-owned Carrot Medical C-View Pro Systems. The contractor shall include all preventive and corrective maintenance required to ensure continuous and optimal performance of the complete C-View Pro System including software, display monitors, suspension equipment, PC and controllers.
