Proposal Information II. Project Description III. Specific Requirements IV Construction Requirements V. Proposal Form VI. Evaluation Criteria I. Proposal Requirements Project: To rebuild the road at the Glenmont Job Corps Center Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator: Adams and Associates, Inc Mailing address Glenmont Job Corps Center 822 River Road Glenmont, NY 12077 Proposal Due Date: 3 PM on 04/01/2014 Payment: Payment Terms are Net 30 days of completion and receipt of Invoice. Site Visit Date: 03/25/2014 at 10:00 AM a) Contractor is mandated to visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # GL-1415 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. b) This proposal shall be valid for a period of 90 days. II. Project Description Scope of Project: The roads at the Glenmont Job Corps Center are deteriorated due to extreme winter weather and recent construction activities in the construction of new dormitories. The long winter freeze and snow caused the deterioration of road surfaces throughout the center and created pot holes that make driving difficult on center roads. The contractor is to provide 4,500 SY of road rebuild (demolish and rebuild gravel base, and install 5" hot mix pavement). Repair the potholes with plant-mix asphalt concrete (approximately 88 SF) and provide 2" hot mix overlay on roads that have a suitable base (approximately 13,400 SY) III Specific Requirements: The project is to be discussed and the work scheduled coordinated with the center throughout this project. Warrantees: a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached prevailing wage rate) ALBANY; COLUMBIA; FULTON; GREENE (that portion north of the line following the south limits of the city of Catskill); MONTGOMERY; RENSSELAER, SARATOGA, SCHENECTADY; SCHOHARIE; WASHINGTON. Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Glenmont Job Corps Center as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for work. The project shall be completed within 120 calendar days of the award of the contract. Written authorization is required to extend the 120 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. Center Finance Office must also have a completed Standard Form 1413, a Signed Waiver and Release of Claims and certified payroll for the time period of the work performed V. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID #GL-1415 and address all items in sections II & III, to be considered. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use. References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9. VI. Evaluation Criteria Evaluation Criteria Point Value PRICE: Responsible Contractor who can perform effective services at the lowest reasonable price 60 QUALITY: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints, 20 PAST PERFORMANCE: Past services to center or list of references for verification purposes 10 SCHEDULE: Detailed explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Lowest price will not guarantee contract award.
