- DEPARTMENT OF VETERANS AFFAIRS REQUEST FOR INFORMATION ON LEGIONELLA PREVENTION PROGRAM The Department of Veterans Affairs is requesting information in order to establish the criteria for a contract to provide our VISN 18 medical facilities (Phoenix, Tucson and Prescott Arizona; Albuquerque, New Mexico; El Paso, Amarillo and Big Spring, TX) with contractors qualified to perform work associated with our legionella disease prevention program. Below is a high-level description of the scope of work being considered, to include additional details as necessary. NCO 18 is looking to establish: 1.SCOPE OF WORK ELEMENTS (High-Level Description) "SCHEMATICS: Provide Schematic (single line) diagrams of the site distribution and domestic water systems (hot and cold) "LEGIONELLA HAZARD TYPE ANALYSIS: Provide a legionella hazard type analysis for the site and specific buildings included in the legionella prevention plan program. "LEGIONELLA AND SCALD PREVENTION PLANS: Provide a packet of information for the prevention plan for each building in the program, and a proposed draft plan for each building. "WATER TESTING: Provide cost per test pricing for typical environment water testing for legionella. "BEST PRACTICES SUGGESTIONS/RECOMMENDATIONS: Provide a list of "lessons learned" or "Best Practices" associated with legionella and scald prevention and control. "WATER DISTRIBUTION SYSTEM ANALYSIS: Include in the legionella hazard analysis or as a separate document a narrative description of the water system to include an assessment of risks or suggested changes/improvements to the water distribution system at each site. "OPTIONAL 2 FOLLOW-UP YEAR OF PROGRAM REVIEW: Review of Hazard Analysis program protocols and their effectiveness quarterly during years 2 and 3. 2.ADDITIONAL DETAILS ON SCOPE ELEMENTS A.Schematics Requirement: The schematic single line diagrams are to present main areas of water distribution and processing and identify any areas in which water is processed differently (e.g., hemodialysis and sterile processing. They should include the following information: "Piping (including type and sizing) - distribution, recirculation and branch line. "Water Storage Tanks (incl capacities and storage temperatures) "Valves (main isolation, tempering, mixing) "Chemical Feed Tanks (chemical, feed concentration, quantities) "Heat exchanges (types, capacities, output temperatures, controls description) "Pumps (feed and circulating, capacities, controls description) "Water outlets (showers, tubs, faucets, eyewashes, urinals, water closets, feeds to equipment such as food service equipment and ice machines "Cross-connects (locations) "Dead-legs (know locations and approximate extents) "Automatic water system drains "Thermometers/thermostats/temperature probes "Expansion tanks "Backflow devices "Water Sampling ports "Chemical Feed Injection Ports "Monitoring Probes and Equipment (temperature and chemical) "Filters "Disinfection process access points "Supplemental water treatment systems (A partial of a schematic line diagram is provided as an attachment) B.Legionella Hazard Type Analysis The model for the analysis provided in ASHRAE STANDARD 188P, Prevention of Legionellosis Associated with Building Water Systems, Draft, may be referenced as a guideline for the expected analysis. C.Legionella And Scald Prevention Plans: Each building in the legionella protection program is to have its own Protection Plan. To support this effort, contractor shall provide a packet of information for the prevention plan for each building to include the elements described below. Also provide a draft plan for each building: a.Schematic single line diagram associated with that building b.List of areas within the building housing in-patients and water system features identified as 'heightened risk' locations. c.Narrative description of water system (main components, age, condition ¦.) d.Primary Control Strategies (existing, suggested best practices) e.Supplemental Control strategies (existing, suggested best practices) f.Supplemental Control strategies (existing, suggested best practices) g.Interim Strategies (pending implementation of any changes to primary strategies based on recommendations or existing plans) h.List of Critical Control Points for the building i.Limits associated with critical control points j.Locations of critical control points shown on schematic single line diagram and process flow diagrams from hazard analysis. k.Description of parameters monitored, by whom, at what frequency, what locations, method of recording, documenting and storing data along with recommended changes/enhancements/best practices l.Evaluate equipment and current processes against equipment and processes needed to comply with draft of national policy. If changes to the water system or program are recommended provide cost estimates m.Inventory of mixing valves n.Inventory of fixtures, ID infrequent usage fixtures o.Any known dead legs based historical information and the walkthrough survey p.Description Remediation plan and evaluation of ability to execute it successfully q.Maintenance plan - narrative summary of what is done and brief description of work. r.Validation Plan 3.INFORMATION REQUESTED Interested contractors are requested to provide information identifying their qualifications to address the scope of work elements noted above, along with a narrative describing, in general, the resources they might use and level of effort required to perform each element of the proposed scope of work. It must be noted, these SOW elements above are not cast in stone, as this is a work- in-progress, subject to revision as the VA develops a better understanding of the Legionella disease prevention and management process. An Opportunity Showcase will be held on Wednesday, March 19, 2014 from 9:00 am until 4:00 pm at the SAO West VISN 18 Network Contracting Office (NCO) located at 4135 S. Power Road, Suite 103, Mesa, Arizona 85212. The showcase will provide interested vendors an opportunity to introduce themselves to the VA and present their proposed strategy to meet the needs of this pending procurement. A 45 minute time slot will be provided for each individual vendor.
