- Attachment #4 - Wage Determinations Attachment #3 - PWS Attachment #2 - MedGas Clauses Attachment #1 - Bid Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. Award will be made using Simplified Acquisition Procedures in accordance with FAR 13.105(b) This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) and the reference number is F2MTW73168A002. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72 and DFARS change notice 20140228. The Offeror must be an authorized dealer and must quote on an all or none basis. Written quotes are required (oral offers will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. The Government will award a contract resulting from this solicitation to the responsible offeror, whose offer is the best value and the most advantageous to the Government which is conforming to the solicitation. Please complete the attached Bid Schedule (Attachment #1) to submited quotes, to include CAGE Code, DUNS, Tax Identification Number. Clauses and Provisions found in Attachment #2 are applicable to this combined synopsis/solicitation and full text and additional information can be found at http://farsite.hill.af.mil. Eligible quotes must be valid through 24 July 2014. This is a 100 percent Small Business set aside. The NAICS Code is 325120 and the standard is 1000 employees. SERVICES REQUIREMENT: This is a non-personal service requirement for Medical Gas Cylinder Rental Services for the operation and administration of several sites for medical activities which include contractor labor, transportation, tools, equipment and materials for medical gas services at Military Treatment Facilities (MTF) in San Antonio, TX. Locations include Wilford Hall Ambulatory Surgical Center (WHASC) and Reid Clinic at Joint Base San Antonio (JBSA), North Central Federal Clinic, Camp Bullis and any successor organization(s) of WHASC, if applicable. See attached Performance Work Statement for description of services - Attachment #3. The Contracting Agency intends to award without discussions but reserves the right to conduct discussions if deemed necessary. The Government will award a purchase order resulting from this Request for Quote (RFQ) to the responsible offeror whose timely quote conforming to the RFQ will be the most advantageous to the Government on the basis of the lowest evaluated price of the quote. Failure to comply with the instructions in the RFQ and/or to quote on all CLINs will result in the agency finding the quote unacceptable and ineligible for award in the absence of discussions. Price will be evaluated for reasonableness in accordance with one or more of the proposal analysis techniques described under FAR 15.404-1, which includes but is not limited to, price analysis and unbalanced pricing. If at any time during this competition the Contracting Officer determines that adequate price competition no longer exists or that price reasonableness cannot be determined, offerors may be required to submit data other than certifies cost or pricing data, as appropriate, for the Contracting Officer to determine price reasonableness. Quotes conforming to Instructions to Offerors to this combined/synopsis solicitation must be emailed to and received by the addressees named below and received not later than 24 March 2014, 1000 AM Central Time. All questions for Q&A must be presented No Later Than 20 March 2014, 1000 AM Central Time to guarantee answering.
