Description

The Contractor shall provide the following Adult Day Health Care (ADHC) Services to all eligible veterans, Extended Care Service Line, or his/her designee, of VA Northern Indiana Healthcare System (VA NIHCS). The Contractor shall provide all necessary personnel, equipment, and supplies associated with the full range of ADHC services in performing the requirements of this contract. Contractor's facility shall be located in the Primary Service Area of VA NIHCS, in order to service veterans residing in the area.
