Sources Sought Document - Franklin, LA This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (hubzone, 8(a), small, small disadvantage, or service disabled veteran owned small business) relative to NACIS 621498, All Other Outpatient Care Centers (size standard of $19 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. All responses to this synopsis are due no later than March 31, 2014. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the Fedbizopps website. Responses to this sources sought synopsis must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (2) Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged business, Service Disabled Veteran Owned small business, Veteran Owned small business, large business. (3) Does your company have experience with the Department of Veterans Affairs capitated CBOC Program? (4) What supplies/services does your company primarily provide? (5) Will your company be a joint venture with another company? (6) How long has your company existed? All interested offerors will have to respond to the solicitation announcement, in addition to responding to this sources sought announcement. The Southeast Louisiana Veteran Healthcare System (SLVHCS) intends to procure Primary Care, Women's Care and limited Mental Health services with space and sufficient administrative support for a Community Based Outpatient Clinic (CBOC). Services provided will include: Primary and Women's Care provided by 0.4 Nurse Practitioner (on site 2 days per week) must be qualified as a Designated Women's Health Provider. 1.0 Collaborating Physician (off site, but available by phone or CVT) Clinical (PACT) support provided by 1.0RN 1.0 LPN 1.0 Clerical Associate Mental Health Care provided by 1.0LCSW Pharmacy Services provided by 0.4 Clinical Pharmacy Specialist (on site or by phone or via CVT) Dietician services provided by 0.2 Registered Dietitian (on site or by phone or via CVT) Housekeeping Security The CBOC is required to be located in Franklin, Louisiana. This CBOC shall be solely dedicated to providing services to Veteran beneficiaries of the Southeast Louisiana Veteran Health Care System. The Contractor shall provide the facility, clinical staff, administrative support, equipment, and labor necessary to support the clinical environment. The NAICS code for this procurement is 621498, All Other Outpatient Care Centers with a small business size standard of $19 Million. The Government intends to award a contract for services for Franklin, Louisiana CBOC. The resultant contract will be firm fixed price indefinite delivery/indefinite quantity with a one-year base period and four (4) one-year option periods. The anticipated solicitation posting date is on or about May 1, 2014. All questions regarding this solicitation shall be communicated in writing via email to the Contract Specialist, Gilda M.
