- I. Scope of Work The Philadelphia VA Medical Center, located at 3900 Woodland Avenue, Philadelphia, PA 19104, is soliciting for a vendor to provide a patient transport chair similar to the Stryker Prime TC Transport Chair that are highly maneuverable, ergonomically constructed to provide greater comfort for patients and reduce work related injuries to staff, and reduces space required to store multiple chairs in one location. These chairs will replace standard wheel chairs throughout the medical center's designated clinical areas. Transport chairs provided at the time of purchase shall be the most recent model available. II. Contractor-Furnished Equipment and Supplies The contractor shall furnish all equipment and supplies necessary to provide patient transport chairs, which meet the criteria established in Section III "Performance Requirements". III. Performance Requirements A.Requirements The vendor shall provide all labor, parts, travel, tools, and equipment to perform all inspections, testing, repair, replacement, and maintenance as required, to maintain full functionality of the patient transport chairs during the warranty period and in accordance with the original equipment manufacturer requirements and recommendations. The warranty period should be a minimum of three years. The vendor will develop a plan to address repairs after normal duty hours, weekends, and holidays to allow for continuous use of the transport chair. All transport chairs provided by the vendor shall meet the general requirements of the Stryker Prime TC Transport Chair, meet or exceed all safety codes, and all other applicable federal, state, and local codes. 1.Specifications : a.Transport chairs shall be highly maneuverable with swivel casters having a diameter of 5-6 inches allowing for ease in pushing, pulling, and maneuvering around objects b.Transport chairs shall be easily stored without folding reducing the storage space in clinical areas and offices where multiple chairs will be stored. An example is: three chairs should be able to be stored in an area 72 inches (6 feet) in length by 28 inches in width. c.Transport chairs shall be constructed in a manner to accommodate a maximum load of 350 US pounds. d.Seat width shall be at least 21.5 inches. e.Seat height should be approximately 21 inches from the floor and 15 inches from the flip-up footrest. f.Chair height with IV Pole should be approximately 72-74 inches. g.Push handles should be approximate 35 inches to 45 inches from the floor and be approximately 16 inches to 21 inches apart. h.Transport chair brakes shall be easily accessible by medical staff without bending to prevent work related injuries. i.IV poles, upright oxygen tank holders, and foley bag hooks shall be included in the vendor's quotes. 2.Education. Education/Training Plan shall be provided to employees primarily designated to operate as well as supervise VA staff as well as the Engineering staff responsible for repairs and periodic maintenance after warranty periods have expired. 3.Warranty and Service a.Specify annual cost for extended maintenance for each transport chair beyond normal warranty period including all contract options. b.Provide suggested maintenance plan which addresses Contractor procedures for repairs, adjustment and replacement as well as after hours and weekend response during warranty period. c.Vendor shall provide a service contact telephone number to Engineering for 24/7 service support. d.Vendor shall provide recommendation for on-site spare parts inventory appropriate to the type and quantities of equipment anticipated to be purchased Where any requirements contained herein appear to be in conflict, the more stringent requirements shall apply. B.Point of Contact (POC) The POC for this project is Judy Echols, Contract Specialist, PVAMC, 3900 Woodland Ave., Rm. A853A, Philadelphia, PA 19104, (215) 823-4652. Faxes or emails will not be accepted. Manuals The contractor shall provide one (1) copy of an owner's manual for each device. The contractor shall provide five (5) copies of the repair manuals to Engineering for use by the designated section responsible for inspection, maintenance and repair. C.Warranty The contractor shall provide at least a three year manufacturer's warranty on all parts and labor. The warranty shall include all travel and shipping costs associated with any warranty repair. **NOTE: THIS INFORMATION IS BEING GATHERED FOR INFORMATIONAL PURPOSES ONLY. THIS IS NOT AN ORDER.
