- Performance Work Statement COMBINED SYNOPSIS/SOLICITATION (I) This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quote (RFQ). Submit written quotes on RFQ reference number AG-7604-S-14-0022. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. (IV) This solicitation is being issued with the associated NAICS code is 811219. The size standard is $19 million. This is not a Small Business Set-A-Side. (V) This combined synopsis/solicitation is for the following commercial item: This acquisition is for the continued FY 2014 - 2016 reoccurring services cost associated with the annual repair and calibration of 151 pieces of Aeroflex test equipment across the nation which fluxes based on lifecycle retirement of older equipment and purchase vs. repair determinations of non-serviceable equipment. This requirement will have a single source which will repair and calibrate all Forest Service Aeroflex test equipment. This contract will keep all equipment in top working condition to perform radio maintenance across the nation. Contractor shall include all labor, materials, supplies, and equipment necessary to provide repairs and calibration maintenance on Aeroflex test equipment to USDA Forest Service (FS) as described in the PWS and in accordance with the terms and conditions listed herein. Prices are all inclusive to include (but not limited to) all items and conditions identified in this document and any associated shipping and handling to and from the FS. 1.2 SERVICES REQUIRED. 1.2.1 The contractor will provide repair and calibration for all 151 pieces of test equipment for one year with the 2 addition option years if exercised. 1.2.2 Provide toll free technical support Monday-Friday, 8:00 am-5:00pm MT. 1.2.3 Provide full maintenance and calibration support per year for all equipment listed in the TECHNICAL EXHIBIT 4. Documented all repairs on return invoice. 1.2.4 Up to 4 calibrations, as needed, a year per piece of equipment with documented calibration levels test results on return invoice. 1.2.5 Return Repaired equipment to the customer within 14 days of the outgoing mail date noted by the FS. 1.2.6 Return Calibrated equipment to the customer within 7 days of the outgoing mail date noted by the FS. 1.2.7 Provide all OEM repair parts and materials for all repairs and calibrations. 1.2.8 All repairs and calibrations shall comply with manufactures specifications and procedures which result in no returns due to contractor's defective repairs or calibrations. 1.2.9 Provide free shipping (2 day service) from the contractors repair facility including shipping containers, packing, and labels. 1.1.1 Any equipment deemed unserviceable or beyond repair capability will be identified with a narrative justification. (VI) Description of requirements is as follows: See attached Performance Work Statement (PWS). (VII) Place of service is at the contractor's facility. The customer's main location is USDA Forest Service Chief Information Office, Albuquerque, New Mexico, 87102. Required services are for 3 years starting March 24, 2014. (VIII) FAR 52.212-1, Instructions to Offerors - Commercial Items (Jun 2008), applies to this acquisition. In addition to written quotes, offerors are instructed to provide a capability statement describing qualifications to provide required services in the PWS. (IX) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), applies to this acquisition. Paragraph A of this clause is completed as follows: 1) Technical, 2) Past Performance, and 3) Price shall be used to evaluate quotes. Price is greater than Technical and Past Performance when combined. (X) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Oct 2010). A copy of this provision is located at http://farsite.hill.af.mil/ (XI) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Jun 2010), applies to this acquisition. All offers for other than Data Center Services will be evaluated as informational only and will be given consideration as required. (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Oct 2010). (XIV) The Government intends to award a Firm Fixed Priced contract resulting from this solicitation, 14 days after posting. The place of performance of this pending contract is USDA Forest Service Chief Information Office, Albuquerque, New Mexico, 87102. (XV) Quotes are required to be received no later than 10:00 am EST, March 21, 2014.
