Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for commercial items, in conjunction with far subpart 13 and as supplemented with additional information included in this notice. This is a Total Small Business Set-Aside requirement only qualified offerors may submit bids. Solicitation VA-249-14-Q-0228 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. NAICS Code is 238220 and size standard is $14.0M Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. Wage determination WD 2005-2495 Revision 16 dated 06/19/2013 applies to this request and can be accessed at the below link: http://www.wdol.gov/wdo/scafiles/std In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov. Description of Services: This requirement is for the VAMC Memphis. The contractor must provide labor, parts, and services to remove existing purge air & combustion air flow switches from boilers # 1, 2 & 4, install new purge air & combustion air flow switches and hardware on boilers #1, 2 & 3, and upon completion of the installation of the new purge air and combustion air flow switches, the contractor will test boilers #1, 2 & 4 to ensure all cutoff switches and alarms function properly. The contractor must be Hayes Cleveland Trained and Certified and documentation illustrating this must be submitted with quote. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to the lowest price quote which conforms to the requirements within this solicitation. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit quantity, and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA documents in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable to shipping charges. STATEMENT OF WORK INTRODUCTION: This requirement is to remove & replace Boilers #1, 2 & 4 Purge Air & Combustion Air Flow switches. BACKGROUND: The existing Purge Air & Combustion Air Flow Switches are outdated and have started to fail. The purge air & combustion Air flow switches are used in conjunction with the burner management system as a safety device to prevent operating the boiler with inadequate air flow in the fire box. SCOPE OF WORK: A Hayes Cleveland Trained and Certified Contractor shall furnish all labor, travel, supervision, materials, and parts to remove, replace, and calibrate Boilers #1, 2 & 4 and Purge Air & Combustion Air Flow switches. REQUIREMENTS: "Hayes Cleveland Trained and Certified "Contractor shall remove existing purge air & combustion air flow switches from boilers # 1, 2 & 4. "Contractor will then install new purge air & combustion air flow switches and hardware on boilers #1, 2 & 3. "Upon completion of the installation of the new purge air & combustion air flow switches, the contractor will test boilers #1, 2 & 4 to ensure all cutoff switches & alarms function properly. "Replace combustion air proving and hi-purge proving switches. "Switches must be piped to comply with "BEI". "Contractor will provide all air switches, valves, and tubing required for the requirement. PLACE OF PERFORMANCE: VAMC Memphis 1030 Jefferson Ave. Memphis, TN 38104 PERIOD OF PERFORMANCE: The requirement is for the job to be completed by 28 March 2014 HOURS OF OPERATION: The contractor will perform services required during the hours of 8:00 am - 4:00 pm, Monday through Friday. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a VAMC Memphis issued identification badge while performing work on any VAMC property. Smoking is not prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. Contractor employees (crew) when on VAMC Memphis grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the VAMC Memphis. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition all company staff assigned to work at the VAMC Memphis. SITE REGULATIONS: All work shall be performed in accordance with VAMC Memphis rules & regulations. WORKING CONDITIONS/SAFETY: The contractor/vendor is solely responsible for working conditions on the job site, including safety of all persons and property during performance of work, and for compliance with OSHA regulations.
