Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format Subpart 12.6, as supplemented with additional information included in this notice. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72(EFFECTIVE 01/01/14). This is a firm fixed priced contract. Only qualified offerors may submit bids. Solicitation VA248-14-Q-0593 is issued as a Request for Quote. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This procurement is set aside for Small Business. The associated NAICs Code is 621511 and size standard is $30 million. Any firm that does not meet the capability and sized standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in System for Award Management (SAM) at www.sam.gov and be capable of furnishing all services listed in the statement of work. A.STATEMENT OF WORK Period of Performance (POP): The base period of performance is from April 1, 2014 plus four (4) one -year options (if all options are exercised, ending 2018. Options may be exercised unilaterally by the Government. Base Year- 1 April 2014 to 30 September 2014 Option Year 1- 1 October 2014 to 30 September 2015 Option Year 2- 1 October 2015 to 30 September 2016 Option Year 3- 1 October 2016 to 30 September 2017 Option Year 4- 1 October 2017 to 30 September2018 Description of Services: Contractor shall provide services to conduct Laboratory testing for water samples to the VA Caribbean Healthcare System, Hemodialysis Unit at Calle Casia, San Juan, Puerto Rico, 00921-3201, two (2) times a year. Contractor shall retrieve the samples collected by the local Medical Instrumentation Technicians (MIT) Dialysis and process the samples for the substances specified in the table below as per the American National Standard (ANSI) and Association for the Advancement of Medical Instrumentation (AAMI)/ ISO 139559:2009, approved in 2011 by the Association for the Advancement of Medical Instrumentation (AAMI). Contractor will provide all necessary supplies as requisition forms and specimen containers; transport samples in such a manner as to ensure and protect the integrity of the specimen; analyze samples. Provide to the VA Caribbean Healthcare System Laboratory Service with a means of communication to permit immediate inquiry regarding the status of pending tests; turnaround time of test results should not be more than two (2) weeks and shall be received by mail. The testing of water samples need to be tested every six (6) months if limits are met and additionally if corrections are required. To comply with dialysis regulations, purified dialysis water needs to be tested to verify it does not exceed the maximum limits of contaminants determined by AAMI (Association for the Advancement of Medical Instrumentation). Contractor shall advice the VA Caribbean Healthcare System of any changes in methodology, procedure, reference ranges and any new test introduced. Compliance with the requirements listed in Table 1 can be shown in one of three (3) ways: 1.Where such testing is available, the individual contaminants in Table 1 can be determined using chemical analysis methods referenced by the American Public Health Association, methods referenced by the U.S. Environmental Protection Agency, and/or other equivalent analytical methods. 2.Where testing for the individual trace elements listed in Table 1 is not available, and the source water can be demonstrated to meet the standards for potable water as defined by the World Health Organization (WHO) or local regulations, an analysis for total heavy metals can be used with a maximum allowable level of 0.1 mg/l. 3.If neither of these options is available, compliance with the requirements of Table 1 can be met by using water that can be demonstrated to meet the potable water requirements of the World Health Organization (WHO) (or local regulations and a reverse osmosis system with a rejection of > 90 % based on conductivity, resistivity or TDS. Samples shall be collected at the end of the water purification cascade or at the most distal point in each water distribution loop. Accordingly, any contract resulting from this solicitation will include these clauses. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) to assist in assessing contractor compliance with the subcontracting commitments incorporated into the contract. To that end, the support contractor(s) may require access to the contractor's business records or other proprietary data to review such business records regarding contract compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Lowest Price technically acceptable is the chosen source selection process. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non- cost factors. The Technical evaluation factors consist of one (1) separate element: Experience. Technical proposals will be rated either acceptable or unacceptable. In order to receive an acceptable rating, the Offeror needs to show that they have technical expertise to perform services listed in the statement of work. Offers that do not provide enough detail experience will be rated as unacceptable. Price will be evaluated for completeness and reasonableness. A determination will be made on whether each price offer is complete, and whether the proposed prices are reasonable. Incomplete or unreasonably priced offers will not be considered for award. The Government may use various price analysis techniques and procedures to make a price reasonableness determination.
