- Dept. of Labor wage determination 05-2023 Rev 17. Hazardous material questionaire and reporting procedures. Concessionaire quote. Non-Appropriated Funds general and special provisions. Statement of work to be performed. •1. The 56 Contracting Squadron, Luke AFB, AZ, has a requirement to operate a Non- Appropriated Fund (NAF) Woodcrafter concession inside the Luke Arts & Crafts Hobby Shop located on Luke Air Force Base in accordance with the attached Statement of Work (SOW). This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number LUKEF071130096 is issued as a Request for Quotation (RFQ). This requirement is unrestricted. The associated North American Industry Classification System Code (NAICS) is 337212 with a size standard of 500. This combined synopsis/solicitation is NOT to be construed as a commitment by the United States Air Force. The Government is NOT liable for any costs and/or activity associated with providing a response to this announcement. Any costs incurred as a result of a response to this announcement shall be borne by the firm and will not be charged to the Government for reimbursement. •2. The Clauses, General and Special Provisions, Articles (Concessionaire Contract) and Wage Determination applicable to this solicitation and the resultant contract are attached. Federal Acquisition Regulation (FAR) clauses can be viewed full text through Internet access at the FAR site: http://farsite.hill.af.mil. Wage Determination WD 05-2023 (Rev 17) applies. Note: In accordance with labor laws, this WD may be updated on the bi-annual anniversary of the contract. •3. Description: The purpose of this contract is to provide a concession within the Luke Arts & Crafts Hobby Shop for a base period to begin on or about 01 Apr 2014 through 31 March 2015 followed by five (5) possible extensions/option years. The contractor will pay the NAFI a minimum guaranteed percentage of a minimum of 20% of all gross monthly sales, minus any sales tax per month for the right to operate a woodcraft concessionaire inside the Luke Arts & Crafts Hobby Shop. Payment to NAFI will be due on the fifth (5th) day of every month. •4. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation provides the best value to the Government. Best Value will be determined according to the following factors which are of equal importance: (i) The Government is expecting a percentage return on the gross sales under the concession. The highest percentage of return offered may be the determining factor in contract award. (ii) Experience (Past Performance): A minimum of three (3) years' experience operating a woodcraft concession or similar endeavor is required. Along with the quote/proposal, the Contractor shall submit a list of at least one but not more than three of the most relevant contracts performed for Federal or commercial agencies within the last three (3) years to include names and telephone numbers of individuals who can be contacted to discuss the information provided. (iii). All Formal questions that reference this solicitation # LUKEF071130096 shall be submitted to Keith Carson, Contracting officer e-mail: keith.carson.2@us.af.mil prior to 13 March 2014. All questions and answers will be posted in FebBizOpps.Gov. •5. NOTE: A 15-day phase-in period is anticipated from time of contract award to commencement of performance and respective work under the contract. Separate Contract Line Items (CLIN"s) are not prescribed to propose or reimburse costs related to phase-in or phase-out. Associated costs for performance related to phase-in and phase-out shall be borne by contractor. It is the incoming contractor's responsibility to work with the outgoing contractor to ensure a smooth and complete transition. •6. Interested parties capable of meeting this requirement MUST submit a written quote to include their tax identification number (TIN) and DUNS and CAGE code as applicable. Oral quotes will not be accepted. All responses that meet the criteria contained within will be considered. Written quotes must be received no later than 13 March 2014, 2:00 p.m. Mountain time and reference the solicitation listed above. Late quotes/offerors will not be considered.
