Description
CENTRAL ALABAMA VETERANS HEALTH CARE SYSTEM #619 BIOMERIEUX BACT/ALERT 3D BLOOD CULTURE SYSTEM STATEMENT OF WORK 619-14-2-7113-0046 Services to provide a "Full service business hours" preventive maintenance plan on the following equipment located at Central Alabama Veterans Health Care System, 215 Perry Hill Road, Montgomery, AL 36109. Services shall include repairs and one (1) preventive maintenance inspection. Period of performance is March 1, 2014 through February 28, 2015. "Biomerieux BacT/Alert 3D Combo Analyzer, Microbiology, Model BacT/Alert 3D combination, EE#51129, Serial #505741, located at 1D106-001-MO, Central Alabama Veterans Health Care System, Montgomery, AL REQUIREMENTS: SCHEDULE OF INSPECTIONS: One inspection of equipment will be made during the first month of the contract. Engineering Service will be notified five (5) days prior to date of inspection. Scheduled inspections will include, but not be limited to, cleaning, lubrication, functional tests and leakage tests. Contractor will provide a written description of the procedures to be performed on each equipment item. REPAIRS: All labor, travel, expenses and parts will be furnished for an unlimited number of telephone support calls and on-demand service visits. NOTE: The basic software configuration and the system-supplied terminal and printer are covered in this agreement. PERFORMANCE: All work will be performed by competent technicians, experienced and qualified to work on listed equipment. Contractor must provide documentation of training technicians have received on this particular equipment. Contractor must provide documentation on all test equipment used to check and calibrate the equipment. PARTS: Contractor shall provide all necessary replacement parts for maintaining the equipment at no cost to the Government. All replaced parts shall become the property of the contractor. EXCLUSIONS: Contractor is not liable for repair, replacement or maintenance arising from (1) any intentional acts or negligence of government employees; (2) attempts to repair or service the equipment made by persons other than Contractor personnel without prior approval of contractor; (3) misuse of the equipment, including use of the equipment for any application or function for which it was not designed. HOURS OF WORK: All work required in the performance of this contract shall be performed during the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday, excluding holidays. REPORTING PROCEDURES: The servicing technician shall report to the Bio-Med Section at the beginning of each visit. After completion of each service visit, including emergency repairs, the servicing technician shall obtain the Engineering Service Representative's signature on the service report as verification of work completed. A copy of all signed service reports shall be left with the Bio-Med Section. In the event work is completed after normal duty hours, the verified report is to be delivered to Engineering by the area supervisor or Engineering representative the following work day. The C&A requirements do not apply and a Security Accreditation Package is not required. INVOICES: All invoices should be submitted in arrears and must contain the appropriate purchase order number. Invoices shall be mailed to VA FSC, PO Box 149971, Austin, TX. 78714. NOTE: If invoice is submitted improperly (too early, incorrect purchase order number, incorrect billing, etc), invoice will be returned to contractor for correction.
