- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (i) This solicitation is issued as a request for quotation (RFQ). (ii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. (iii) This is a small-business set aside. Quotations are being solicited from small businesses only. The NAICS code is 541711 and the small business size standard is 500 employees. (iv) CLIN Item Quantity Unit 0001 Stability Extension 1 job (v) See the attached statement of work for a description of requirements for the service to be acquired. (vi) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Jul 2013), applies to this acquisition and is incorporated herein by reference. (vii) The Government will award a contract resulting from this solicitation to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotations: (i) Technical capability of the quotation to meet the agency need, (ii) Price. (viii) Quoters MUST complete and return a copy of the representations and certifications in the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Nov 2013), incorporated herein by reference. (ix) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Sep 2013), applies to this acquisition and is incorporated herein by reference. (x) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jan 2014), applies to this acquisition and is incorporated herein. The following additional FAR clauses cited in the clause are applicable to the acquisition: Paragraphs (b)14, 25, 28, 30, 31, 33, 40, 44. (xi) The clause at HHSAR 352.222-70, Contractor Cooperation in Equal Employment Opportunity Investigations (January 2010), applies and is incorporated herein by reference. (xii) To be evaluated for award, a quoter must submit a quotation via email with the title "Stability Extension Quotation" to kevin.nilles@hhs.gov no later than the cut-off time. Quotations shall not exceed 20 pages in length.
