THIS OPPORTUNITY IS AVAILABLE ONLY TO SMALL Businesses. The Lebanon VA Medical Center is soliciting bids under Solicitation No. VA244-13-Q-0494 for the following: Diagnostic Radiology Equipment (annual) it may require 5 visits to complete all 1.Annual evaluation of all X-ray Imaging Equipment including - Radiographic tubes, Fluoroscopic tubes, Mobiles, C-arms, Dental units, and DEXA scanners. 2.Annual evaluation of the CT scanner. 3.Provide annual patient dose tables for Radiographic equipment. 4.Provide annual lead apparel review and inventory. 5.Provide follow up as needed. 6.Provide Annual Radiation safety training/ In-Services as needed. Note: current status of approximately 11 tubes, 13 dentals, and 1 CT unit. The Lebanon VA uses no film, there are no printers. The facility has approximately 100 lead aprons and other protective devices Nuclear Medicine Regulatory Audit Review and Report (Quarterly in MAR, JUN, SEP, DEC) 1.Regulatory management audit review and report (includes, but not limited to, record-keeping audit) - to assure you of continuing compliance with National Health Service (NHS) standards. 2.Test ancillary equipment required for licensure, such as well counters, dose calibrators, and radiation survey meters for proper operation. Calibration schedules will be monitored for survey meters in order to comply with annual calibration requirements provided by separate third party vendors. 3.Provide regulatory update discussions, along with handouts, and in-service presentations (electronic) for continuing education compliance. 4.Provide inventories and radiation surveys of sealed sources as required. 5.Provide semi-annual sealed source leak tests 6.Review personnel monitoring records and make recommendations - as requested. 7.Review staff Q.C. records and make recommendations - as needed. MRI (1 time per year) 1.Annual ACR-level System and Coil testing evaluation and QC review on MRI unit, including full data report and detailed recommendation summary letter. 2.Provide on-going consultation by phone and email to support and assist MRI Technologists with QC requirements and safety issues. The Solicitation documents will be available for download from the web https://www.fbo.gov on or about February 5, 2014. Potential offerors are responsible for monitoring this site for the release of the solicitation package and any other pertinent information. The Government will not provide paper copies of the solicitation. Offerors will be responsible for downloading their own copy of the solicitation package, subsequent amendments, if any. It is essential that interested parties register on the Interested Vendor Listing at https://www.fbo.gov, "Interested Vendor List". Individual copies of the Interested Vendor List WILL NOT be prepared nor distributed by the Contracting Officer. The solicitation amendments issued to the IFB will be posted to the website and bidders are advised it is their responsibility to obtain and acknowledge all amendments. Paper copies of amendments will NOT be individually mailed. All requests for information or questions regarding the solicitation must be submitted in writing to the Contracting Officer via mail or by email: carl.devitz@va.gov no later than Close of Business, February 12, 2014. No telephonic or fax requests will be honored. In accordance with FAR Part 36.204, the magnitude of this project is between $13000 and $40,000. NAICS Code 541690 with a small business size standard of $14 million. The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. The government is also contemplating the use of two option periods for this requirement. Option period will be subject to the availability of funds and continued need for the service.
